Study on 2μm continuous-wave laser enucleation combined bipolar-transurethral resection of the prostate for the treatment of large volume benigh prostate hyperplasia
Abstract:Objective To assess the safety and effectiveness of 2μm continuous-wave laser enucleation combined bipolar-transurethral resection of the prostate for the treatment of large volume benigh prostate hyperplasia. Methods Clinical data between January 2014 and December 2017 of 82 BPH patients whose prostate volume > 80 ml is retrospectively analyzed. The age ranged from 58 to 84 years old(71.1 ± 5.8y), prostate volume was 81~182(104.4 ± 24.2)ml. Operations were successful and all were given suprapubic puncturecy stostomy. Datas such as operation time, weight of excisedtissue, intraoperative estimated blood loss, postoperative bladder irrigation time, postoperative catheter indwelling time, blood transfusion event, TURS, postoperative urinary incontinence were collected. Blood electrolyte, hemoglobin, the international prostate symptom score(IPSS),quality of life score(QoL) and maximum urinary flow rate (Qmax) before and after operation were compared. Results All operations were successful. Mean operation time was 106.9 ± 30.1 min. Mean weight of excision was 58.5 ± 17.7 g. Mean intraoperative estimated blood loss was 107.6 ± 29.1 ml. Mean postoperative bladder washing time was 45.1 ± 6.3 h. Mean postoperative urethral catheter time was 5.1 ± 1.4 d. There was no statistically significant difference in blood electrolyte and hemoglobin before and after surgery. Statistically significant difference in IPSS),QoL and Qmax was found before and after surgery (P < 0.05). No intraoperative major complications occurred. Postoperative bleeding was happened on one patient who underwent a secondary surgical intervention and blood transfusion. 5 cases had temporary incontinence and 3 had urinary tract infection. no urethral stricture and bladder neck contracture happened. Conclusion 2μm continuous-wave laser enucleation combined bipolar-transurethral resection of the prostate who takes advantage of all the equipments and surgical technique is a safe and effective minimally invasive treatment for large volume BPH.
