Epidemiological analysis of the maintenance hemodialysis patients with acute cholecystitis over the past 5 years

Abstract:Objective We investigated and analyzed the epidemiology of maintenance hemodialysis (MHD) patients with acute cholecystitis in our hemodialysis center,in order to provide data for early diagnosis, optimal treatment and continuous quality improvement. Methods We retrospectively reviewed the clinical records of MHD patients with acute cholecystitis treated for more than 3 months in our hemodialysis center during Jan 2013 to Dec 2017. The factors we measured includes gender, age, causes of kidney disease, clinical manifestations of acute cholecystitis, the incidence, etc. Results In our hemodialysis center, the number of MHD with acute cholecystitis increased annually. Among the 80 MHD with acute cholecystitis patients, 34 patients were males and 46 patients were females with a male/female ratio of 0.74 to 1. The annual incidence was 5.2% ,7.6% ,8.5% ,11.3% and 15.9% respectively. But there is no statistical difference. Most patients with acute cholecystitis were in the age of 60 to 69. The median age was 62.5 years,the minimum age was 44 years and the maximum age was 82 years. The primary disease for the 78 patients with defined causes was diabetic nephropathy (53.8%), hypertensive nephrosclerosis (42.3%),drug-induced kidney damage (2.6%) and polycystic kidney (1.3% ). The duration of hospitalization in MHD patients with acute cholecystitis was 7 to 20 days,significantly longer than that of the acute cholecystitis patients. The symptoms of most patients are not typical, and conservative treatment is relatively safe. Conclusion In our hemodialysis center, the majority of the MHD patients with acute cholecystitis were elderly, females. Primary causes leading to ESRD are diabetes and hypertension. Their clinical symptoms are not typical. The duration of hospitalization is longer than that of the acute cholecystitis patients.
