Application Effect of 3D-Printed UAS Sheath Posterior Device in FURS for Complex Renal Calculi

Abstract:Objective To explore the application effect of 3D-printed ureteral access sheath(UAS)posterior device in flexible ureteroscopic lithotripsy(FURS)for complex renal calculi and its influence on stone clearance rate and complications.Methods A total of 124 patients with complex renal calculi admitted to the First Affiliated Hospital of Henan University from July 2023 to July 2025 were prospectively selected and divided into 3D group(62 cases)and conventional group(62 cases)by random number table method.The conventional group received conventional FURS treatment,and the 3D group received 3D-printed UAS posterior device-assisted FURS treatment.The stone clearance rate,perioperative conditions,preoperative and postoperative 1-day renal interlobar artery hemodynamics[peak systolic velocity(PSV),end-diastolic velocity(EDV),resistance index(RI)],renal injury indexes[β2-microglobulin(β2-MG),neutrophil gelatinase-associated lipocalin(NGAL),cystatin C(Cys C),blood urea nitrogen(BUN),serum creatinine(SCr)]and stress-inflammatory factors[cortisol(Cor),interleukin-6(IL-6),superoxide dismutase(SOD),high-sensitivity C-reactive protein(hs-CRP)],the preoperative and postoperative 6 h,12 h,24 h and 48 h visual analogue scale(VAS)were compared between the two groups.And the postoperative complications were statistically analyzed.Results The one-time stone clearance rate in the 3D group was 91.94%,which was significantly higher than 79.03%in the conventional group(P<0.05).The operation time,catheter removal time and hospitalization time in the 3D group were(61.32±8.19)min,(2.24±0.65)d and(4.97±1.39)d,respectively,all shorter than those in the conventional group[(72.45±10.57)min,(2.86±0.71)d,(6.21±1.48)d](P<0.05).There was no significant difference in intraoperative blood loss or hospitalization expenses between the two groups(P>0.05).At postoperative 1 d,the 3 D group showed higher PSV and EDV,and lower RI of the renal interlobar artery than the conventional group(P<0.05).At postoperative 1 d,the serum levels of NGAL,β2-MG,BUN,Cys C,SCr,Cor,IL-6 and hs-CRP in the 3 D group were lower than those in the conventional group,while the SOD level was higher(P<0.05).The VAS scores at 6,12,24 and 48 h postoperatively in the 3D group were all lower than those in the conventional group(P<0.05).The incidence of complications in the 3D group was 9.68%,which was lower than 24.19%in the conventional group(P<0.05).Conclusion The 3D-printed UAS posterior device-assisted FURS can improve the one-time stone clearance rate in the treatment of complex renal calculi,reduce intraoperative injury,and lower the risk of postoperative complications.
