Comparison of Vascular Drainage Method and CT-Guided Coil Localization for Thoracoscopic Wedge Resection of Lung

Abstract:Objective To investigate the application value of vascular drainage localization and CT-guided coil localization in thoracoscopic wedge resection of lung.Methods A total of 99 patients undergoing thoracoscopic wedge resection of lung in the First Affiliated Hospital of Henan Medical University from January 2023 to December 2024 were selected and divided into drainage group(49 cases)and conventional group(50 cases)according to the random number table method.The drainage group received vascular drainage localization,and the conventional group received CT-guided coil localization.The operation conditions,perioperative conditions,lung function[forced expiratory volume in one second(FEV1),forced vital capacity(FVC),peak expiratory flow(PEF)]before and 3 months after surgery,and complications were compared between the two groups.Results There were no significant differences in the success rate of first resection,the rate of secondary resection,the rate of conversion to thoracotomy,the distance of incision margin,and the drainage volume at 24 h after operation between the two groups(P>0.05).The operation time in the drainage group was(139.52±38.71)min,which was longer than(102.63±34.58)min in the conventional group,and the blood loss was(60.16±13.29)mL,which was more than(36.84±7.51)mL in the conventional group(P<0.05).At 3 months after surgery,there were no significant differences in FEV1,FVC,and PEF between the two groups(P>0.05).The incidence of preoperative localization-related complications in the drainage group was 0,which was lower than 16.00%in the conventional group(P<0.05).There was no significant difference in the total incidence of postoperative complications between the two groups(P>0.05).Conclusion Both localization schemes are effective auxiliary techniques for thoracoscopic wedge resection of the lung.The vascular drainage method can reduce preoperative localization-related complications and has higher safety,while CT-guided coil localization can shorten operation time and reduce intraoperative blood loss.Both have their own advantages and disadvantages,and should be selected according to the specific clinical situation.
