Efficacy of VMAT Radiotherapy in the Treatment of Locally Advanced Cervical Cancer

Abstract:Objective To observe and analyze the efficacy of volumetric modulated arc therapy(VMAT),as well as the acute adverse reactions and dosimetry differences after treatment in patients with locally advanced cervical cancer.Methods A retrospective analysis was performed for 96 patients with locally advanced cervical cancer admitted to Henan Provincial People's Hospital,from April 2022 to March 2024,and the short-term efficacy,acute adverse reactions and dosimetry differences between the two groups were compared according to different treatment regimens,including the reference group[48 cases,receiving intensity-modulated radiation therapy(IMRT)]and the study group(48 cases,receiving VMAT).Results There was no significant difference in short-term efficacy between the two groups(P>0.05),and there was no significant difference in the irradiation dose of V107 and D98% between the two groups(P>0.05),and there was a significant difference in V100,D2%,D50%,D90%,D95%,Dmax,Dmean,uniformity index(HI)and conformal index(CI)(P<0.05).There was a significant difference in the dose distribution of bladder in the V10 parameter(P<0.05),but there was no significant difference in the other organs(P>0.05).In the V20 parameter there was a significant difference in the dose distribution of bladder,pelvis,small intestine,femoral head and rectum(P<0.05).In the V30 parameter,there was a significant difference in the dose distribution of pelvis,small intestine and femoral head(P<0.05).There was no significant difference in rectal dose distribution(P>0.05),bladder dose distribution in V40 parameters(P<0.05),and other organs(P>0.05),and bladder,pelvis,small intestine,femoral head and rectum in Dmax and Dmean parameters(P<0.05).In the reference group,7 cases(14.58%),6 cases(12.50%)developed radiation proctitis,and 11 cases developed bone marrow suppression(22.92%),compared with 5 cases of radiation cystitis(10.42%),4 cases of radiation proctitis(8.33%),and 7 cases of bone marrow suppression(14.58%)in the study group,the complication rate was higher and the difference was not obvious(P>0.05).Conclusion Both IMRT and VMAT radiotherapy can achieve good short-term efficacy,but VMAT has significant advantages in reducing the incidence of acute adverse reactions,and can better protect the target area and threatened organs.
