Risk Factors for Postoperative Bleeding After Endoscopic Mucosal Resection in Patients with Colonic Flat Polyps

Abstract:Objective To investigate the postoperative bleeding after endoscopic mucosal resection(EMR)in patients with colonic flat polyps and to explore its influencing factors.Methods A total of 200 patients with colonic flat polyps who underwent EMR in the Third People's Hospital of Henan Province from June 2023 to May 2024 were selected as the study subjects.Clinical data of the patients were collected,and they were divided into a bleeding group and a non-bleeding group based on whether postoperative bleeding occurred within 3 months after surgery.Clinical data were compared between the two groups,and logistic multivariate regression analysis was used to identify the influencing factors for postoperative bleeding after EMR for colonic flat polyps,followed by ranking of importance.Results Among the 200 patients with colonic flat polyps after EMR,17 developed postoperative bleeding,with an incidence of 8.50%.Univariate analysis showed statistically significant differences between the non-bleeding group and the bleeding group in terms of preoperative antithrombotic medication,maximum resected polyp diameter,intraoperative blood loss,intraoperative electrocoagulation hemostasis,and postoperative recovery diet time(P<0.05).Logistic multivariate regression analysis revealed that patients with colonic flat polyps who received preoperative antithrombotic medication had a 2.088-fold higher risk of postoperative bleeding after EMR than those without preoperative antithrombotic medication;those with a maximum resected polyp diameter>1 cm had a 2.717-fold higher risk than those with a maximum resected polyp diameter≤ 1 cm;those with intraoperative blood loss>50 mL had a 5.346-fold higher risk than those with intraoperative blood loss ≤50 mL;and those with postoperative recovery diet time>48 h had a 0.659-fold higher risk than those with postoperative recovery diet time ≤48 h(P<0.05).The ranking of importance showed that intraoperative blood loss(>50 mL)ranked first,followed by maximum resected polyp diameter(>1 cm),preoperative antithrombotic medication,and postoperative recovery diet time(>48 h).Conclusion Postoperative bleeding after EMR in patients with colonic flat polyps is associated with various factors,including disease condition,medication use,and postoperative diet.Medical staff can optimize controllable variables based on these findings to guide clinical application.
