Predictive Value of Ultrasound Renal Blood Flow Parameters Combined with Serum HMGB-1 and UMOD Levels for Renal Injury in Patients with Essential Hypertension

Abstract:Objective To explore the predictive value of ultrasound blood flow parameters combined with the detection of serum high mobility group box 1(HMGB-1)and uromodulin(UMOD)for renal injury in patients with essential hypertension.Methods A total of 143 patients with essential hypertension admitted to the Second Affiliated Hospital of Zhengzhou University from December 2022 to December 2024 were selected and divided into the renal injury group(52 cases)and the non-renal injury group(91 cases)according to the renal injury conditions.General data such as the patients' gender,age,body mass index(BMI),duration of hypertension,and smoking history were collected.The biochemical indicators such as serum creatinine(SCr),blood urea nitrogen(BUN),triglycerides(TG),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C)of the patients were detected.The renal blood flow parameters such as the resistance index(RI),peak systolic velocity(PSV),and end-diastolic velocity(EDV)of the interlobar artery(IRA),segmental renal artery(SRA),and main renal artery(MRA)of the patients were detected.The levels of serum HMGB-1 and UMOD were detected.Spearman rank correlation analysis was used to evaluate the correlations of renal injury with renal blood flow parameters and serum levels of HMGB-1 and UMOD in patients with essential hypertension.Logistic regression model was employed to identify the risk factors for renal injury,and receiver operating characteristic(ROC)curve was utilized to analyze the predictive value of the above indicators for renal injury.Results The duration of hypertension,IRA-RI,MRA-RI and serum HMGB-1 levels in the renal injury group were higher than those in the non-renal injury group,while the IRA-EDV and serum UMOD levels were lower than those in the non-renal injury group(P<0.05).Renal injury in patients with essential hypertension was positively correlated with the levels of serum HMGB-1,and negatively correlated with the levels of IRA-EDV and serum UMOD(P<0.05).The level of serum HMGB-1 was a risk factor for renal injury in patients with essential hypertension,while the levels of IRA-EDV and serum UMOD were protective factors for renal injury in patients with essential hypertension(P<0.05).The areas under the curve(AUC)of IRA-EDV,serum HMGB-1 level,serum UMOD level and the combined prediction of renal injury in patients with essential hypertension by the three indicators were 0.740,0.728,0.745 and 0.861,respectively.The Youden indices were 0.429,0.341,0.363 and 0.596,respectively.Conclusion The ultrasound renal blood flow parameter IRA-EDV and the levels of serum HMGB-1 and UMOD are the influencing factors of renal injury in patients with essential hypertension.IRA-EDV,serum HMGB-1 level,and serum UMOD level can be used as auxiliary predictive indicators for renal injury in patients with essential hypertension.
