Influencing Factors of Long-Term Patency Rate After Endovascular Volume Reduction Combined with Drug-Coated Balloon in the Treatment of Femoral Popliteal Artery Calcification Lesions

Abstract:Objective To investigate the long-term patency rate and influencing factors of endovascular volume reduction combined with drug-coated balloon(DCB)in the treatment of femoral popliteal artery calcification lesions.Methods A total of 205 patients with femoral popliteal artery calcification lesions in Puyang Oilfield General Hospital from January 2022 to September 2023 were selected as the research objects.All patients were treated with endovascular volume reduction combined with DCB.After 2 years of follow-up,the patency rate was counted.After the 4 patients who were lost to follow-up were excluded,they were divided into patency group(137 cases)and non-patency group(64 cases).The clinical data were collected.The influencing factors of long-term patency rate of femoral popliteal artery calcification lesions were analyzed by single factor and LASSO-Cox regression,and a nomogram prediction model was constructed.Receiver operating characteristic(ROC)curve,calibration curve and clinical decision curve analysis(DCA)curve were used to evaluate the predictive value of the prediction model.Results The primary patency rate of 201 patients at 2 years after operation was 68.16%.The proportion of diabetes mellitus,renal insufficiency,critical limb ischemia(CLI),Trans-Atlantic Inter-Society Consensus(TASC)grade C,popliteal artery involvement,severe calcification and occlusion in the non-patency group were higher than those in the patency group,the lesion length was longer than that in the patency group,and the number of outflow tracts was lower than that in the patency group,the differences were statistically significant(P<0.05).LASSO-Cox regression analysis showed that diabetes,renal insufficiency,CLI,TASC grade C,and lesion length were independent risk factors for long-term patency,the number of outflow tracts was an independent protective factor(P<0.05),and there was no multicollinearity(VIF<5).The nomogram prediction model was constructed,and the Hosmer-Lemeshow goodness of fit test showed that P=0.375,suggesting that the model fit was good.ROC analysis showed that the area under the curve of the model for predicting long-term patency was 0.890(95%CI:0.846-0.934).The calibration curve showed that the consistency index of the model was 0.858 and the calibration was 0.844.The DCA curve showed that the model showed a significant and stable clinical positive net benefit in a wide threshold probability range of 0.15-0.80.Conclusion Diabetes,renal insufficiency,CLI,TASC grade C,lesion length and number of outflow tracts are all independent influencing factors of long-term patency rate of femoral popliteal artery calcification lesions.The nomogram model based on the above factors has good predictive efficacy for long-term patency,and has good discrimination,calibration and clinical practicability,which can provide a new quantifiable tool for clinical accurate prediction and individualized intervention.
