Predictive Effect of C-Reactive Protein-Triglyceride Glucose Index on Disease Outcome After Intravenous Thrombolysis in Acute Ischemic Stroke

Abstract:Objective To explore the predictive efficacy of C-reactive protein(CRP)-triglyceride glucose index(TyG)in the prognosis of acute ischemic stroke(AIS)after intravenous thrombolysis.Methods A total of 98 AIS cases who received intravenous thrombolysis at the Traditional Chinese Medicine Hospital of Xinyang,Gonggu Road Campus from January 2022 to June 2024 were selected.According to the modified Rankin scale(mRS)score of cases,a good prognosis group(mRS score ≤2 points)and a poor prognosis group(mRS score>2 points)were set up.Clinical data of all participants were collected and CRP-TyG(CTI)was calculated.The efficacy of CRP,TyG and CTI in predicting disease outcome after intravenous thrombolysis in AIS patients was analyzed by receiver operating characteristic(ROC)curve,and the influencing factors of disease outcome in AIS patients were analyzed by multivariate logistic regression.Results The levels of CRP,TyG and CTI in the poor prognosis group were higher than those in the good prognosis group(P<0.05).ROC results showed that the AUC(95%CI)of CRP,TyG and CTI to predict disease outcome after intravenous thrombolysis in AIS cases were 0.737(0.692-0.787),0.824(0.779-0.874)and 0.926(0.881-0.976),respectively.The AUC predicted by CTI after intravenous thrombolysis in AIS cases was higher than that diagnosed by CRP and TyG alone(Z=9.472,P<0.001;Z=11.845,P<0.001).The National Institute of Health stroke scale(NIHSS)score at admission,uric acid level and the proportion of bleeding after thrombolysis in the poor prognosis group were higher than those in the good prognosis group(P<0.05).NIHSS score at admission(OR=2.333,95%CI:1.314-4.142),bleeding after thrombolysis(OR=2.073,95%CI:1.233-3.485),CTI(OR=3.725,95%CI:1.958-7.084)were the influencing factors of disease outcome after intravenous thrombolysis in AIS cases(P<0.05).Conclusion The CTI value at admission of AIS patients is an independent influencing factor of adverse disease outcomes after intravenous thrombolysis,which has certain clinical promotion value.
