Correlation Between Serum RLP-C,Adipsin,PCSK9 and the Risk of Recent Disease Progression in Elderly Patients with Chronic Coronary Syndrome

Abstract:Objective To investigate the correlation between serum residual lipoprotein-cholesterol(RLP-C),adipsin,proprotein convertase subtilisin/kexin type 9(PCSK9)and the risk of recent disease progression in elderly patients with chronic coronary syndrome(CCS).Methods A total of 199 patients with CCS admitted to Zhengzhou People's Hospital from May 2023 to June 2025 were selected and followed up for 3 months.According to whether the disease progressed,they were divided into progress group(50 cases)and non-progression group(149 cases).The baseline data and serum levels of RLP-C,adipsin and PCSK9 were compared between the two groups.The effects of baseline serum levels of RLP-C,adipsin and PCSK9 on the progression of CCS were analyzed.The value of baseline serum RLP-C,adipsin and PCSK9 alone and in combination in predicting the progression of CCS was analyzed,and the robustness of the combination of the three in predicting the progression of CCS was verified.Results The scores of treadmill exercise test(Duke)and low density lipoprotein cholesterol(LDL-C)in the CCS progression group were higher than those in the non-progression group,and the differences were statistically significant(P<0.05).The levels of serum RLP-C,adipsin and PCSK9 in the progressive group after 3 months were higher than those at admission(P<0.05).The levels of serum RLP-C,adipsin and PCSK9 in the progressive group were higher than those in the non-progressive group at admission and 3 months after admission(P<0.05).Before and after correction of Duke score and LDL-C,serum RLP-C,adipsin and PCSK9 at admission were all related factors affecting the progression of CCS(P<0.05).The area under the curve of receiver working characteristic(ROC-AUC)of serum RLP-C,adipsin and PCSK9 at admission were 0.752,0.769 and 0.778,respectively(P<0.05),and the combined ROC-AUC was greater than the ROC-AUC values of the three alone(Z=2.306,2.308,2.038,P<0.05).The precision-recall(PR)curve showed that the combined predicted PR-AUC was 0.834.Conclusion Serum RLP-C,adipsin and PCSK9 are related to the risk of recent disease progression in elderly patients with CCS.The combination of various indicators can provide guidance for clinical prediction of CCS disease progression,so as to assist clinical timely intervention and improve the prognosis of patients.
