Predictive Value of AISI for Major Adverse Cardiovascular Events in Postmenopausal Women with Acute Coronary Syndrome

Abstract:Objective To investigate the predictive significance of the aggregate index of systemic inflammation(AISI)for major adverse cardiovascular events(MACE)in postmenopausal women diagnosed with acute coronary syndrome(ACS).Methods A total of 584 postmenopausal women diagnosed with ACS and treated at the Department of Cardiology at the First Affiliated Hospital of Zhengzhou University from January 2019 to January 2021 were included in this retrospective cohort study.Patients were categorized into low-AISI group(292 cases)and high-AISI group(292 cases)based on the median AISI value of 219.5.The primary endpoint was the occurrence of MACE within 3 years,defined as all-cause mortality,myocardial infarction,or target vessel revascularization.Baseline characteristics were compared between the two groups.Kaplan-Meier survival analysis was performed to assess differences in MACE.Cox proportional hazards regression analysis was conducted to identify the predictors of MACE.Receiver operating characteristic(ROC)curve analysis was employed to evaluate the predictive performance of AISI for MACE,and the incremental predictive value of combining AISI with N-terminal pro-B-type natriuretic peptide(NT-proBNP)for MACE was also assessed.Results Compared with the low-AISI group,the high-AISI group exhibited higher prevalence rates of diabetes mellitus,acute myocardial infarction(AMI),and multivessel disease,as well as significantly elevated levels of mean age,fasting blood glucose,glycated hemoglobin,aspartate aminotransferase,creatine kinase-MB(CK-MB),troponin I,and NT-proBNP(P<0.05).During the 3-year follow-up period,101 cases(17.3%)MACE occurred,including 38 cases(13.0%)in the low-AISI group and 63 cases(21.6%)in the high-AISI group,with a statistically significant difference between groups(P=0.005).Multivariate Cox regression analysis revealed that lg AISI was an independent predictor of MACE after adjustment(HR=2.048,95%CI:1.075-3.901,P=0.029).ROC analysis demonstrated that AISI had a moderate predictive value for MACE,with an area under the curve(AUC)of 0.614(95%CI:0.553-0.675,P<0.001).The combination of AISI and NT-proBNP improved the predictive accuracy,yielding an AUC of 0.658(95%CI:0.596-0.720,P<0.001).Conclusion AISI is significantly associated with the occurrence of MACE in postmenopausal women with ACS and serves as an independent risk factor for MACE.It demonstrates moderate predictive value for MACE,which can be improved by incorporating it with NT-proBNP.
