Effects of HoLEP and TURP on Traumatic Inflammatory Stress in Patients with Small Volume Benign Prostatic Hyperplasia

Abstract:Objective To compare the clinical efficacy of transurethral holmium laser enucleation of the prostate(HoLEP)and transurethral resection of the prostate(TURP)for the treatment of small-volume benign prostatic hyperplasia(BPH).Methods From January 2021 to January 2024,90 patients with small volume BPH admitted to Henan Provincial Hospital of Traditional Chinese Medicine were selected for a prospective single blind controlled study.They were randomly divided into a control group(45 cases)and a study group(45 cases)using a random number table method.The control group underwent TURP treatment,while the study group underwent HoLEP treatment.The surgical-related indicators,levels of traumatic inflammatory stress[C-reactive protein(CRP),interleukin-6(IL-6),monocyte chemoattractant protein-1(MCP-1)],urinary function indicators[residual urine volume(RUV),maximum flow rate(Qmax),pressure-flow rate value(A-G value)],International prostate symptom score(IPSS score),benign prostatic hyperpla-sia-quality of life(BPH-QOL),and complications were compared between the two groups.Results The intraoperative blood loss of the study group was less than that of the control group,and the bladder irrigation time,catheter indwelling time and hospitalization time of the study group were shorter than those of the control group(P<0.05).The levels of serum CRP,IL-6 and MCP-1 in the study group were lower than those in the control group at 3 days after operation(P<0.05).At 3 months after operation,RUV,A-G value and IPSS score in the study group were lower than those in the control group,while Qmax,BPH-QOL score in the study group were higher than those in the control group(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion Both HoLEP and TURP have good long-term efficacy and safety in the treatment of small-volume BPH patients,while the former can reduce tissue damage,reduce inflammatory response,and promote early postoperative recovery.
