Clinical Effect of Photodynamic Therapy in Orthodontic Treatment of Patients with Malocclusion Combined with Periodontitis

Abstract:Objective To explore the clinical value of photodynamic therapy in the orthodontic treatment of patients with malocclusion complicated by periodontitis.Methods A total of 116 patients with malocclusion and periodontitis who received orthodontic treatment at the First Affiliated Hospital of Zhengzhou University from January to December 2023 were enrolled and assigned using a computerized random number table into an observation group and a control group,with 58 cases in each group.The control group received conventional comprehensive orthodontic treatment,while the observation group received additional photodynamic therapy on top of standard therapy.Periodontal parameters,masticatory function,inflammatory markers,and periodontal pathogen counts were recorded and compared before treatment and at 3 and 6 months after treatment.Results Both groups showed decreased clinical loss of attachment values post-treatment compared to pre-treatment levels,with the observation group showing lower values than the control group(P<0.05).Post-treatment,the bite force and chewing efficiency scores were higher than pre-treatment,and the observation was higher than the control group(P<0.05).Post-treatment,α-hydroxybutyrate dehydrogenase levels were observed elevated,and soluble intercellular adhesion molecule-1 and monocyte chemotactic protein-1 concentrations decreased(P<0.05).Post-treatment,both Actinobacillus agglomerans and Fusobacterium nucleatum populations declined in the two groups,and the observation group was lower than the control group(P<0.05).Additionally,Porphyromonas gingivalis levels decreased over time in the observation group,but no statistically significant differences were found between the groups(P>0.05).Conclusion Photodynamic therapy,as an adjunct to orthodontic treatment for malocclusion with periodontitis,can improve periodontal health,enhance masticatory function,reduce inflammatory responses,and lower periodontal pathogen loads.
