Correlation Between Serum FGF23,HIF-1α Expression and Pulmonary Hypertension in Hemodialysis Patients

Abstract：Objective To investigate the correlation between the expression of serum fibroblast growth factor 23(FGF23)and hypoxia inducible factor-1α(HIF-1α)and the occurrence of pulmonary arterial hypertension(PAH)in hemodialysis patients.Methods A retrospective analysis was conducted to collect clinical data of 148 patients who underwent maintenance hemodialysis in hospitals for more than 6 months from January 2021 to January 2024.Based on the results of pulmonary artery pressure measured by echocardiography at 6 months of hemodialysis,the patients were divided into PAH group(46 cases)and non PAH group(102 cases).At the same time,the PAH group was evaluated for disease severity based on the average pulmonary artery pressure value and divided into subgroups:mild group(14 cases),moderate group(22 cases),and severe group(10 cases).The data required for the study were reviewed and recorded,and the possible factors were included.Logistic regression was used to analyze the influencing factors of PAH in hemodialysis patients,and the relationship between serum FGF23,HIF-1α expression and PAH in hemodialysis patients was analyzed.Results During dialysis,the increase in body mass,serum FGF23,HIF-1α,blood phosphorus,and total parathyroid hormone levels in the PAH group were higher than those in the non PAH group,and hemoglobin was lower than that in the non PAH group(P＜0.05).Multivariate logistic regression analysis showed that the increase in body mass,FGF23,HIF-1α,hemoglobin during dialysis were associated with the occurrence of PAH in hemodialysis patients(P＜0.05).There was a positive interaction between serum FGF23 and HIF-1α levels in hemodialysis patients and the occurrence of PAH.The levels of serum FGF23 and HIF-1α in the severe group were higher than those in the moderate and mild groups,and the moderate group was higher than the mild group(P＜0.05).Conclusion The abnormal elevation of serum FGF23 and HIF-1α is related to the occurrence of PAH in hemodialysis patients,and there is an interaction between the two indicators.
