Clinical Analysis of 17 Cases of Primary Female Genital Tract Malignant Melanoma

Abstract:Objective To discuss clinical characteristics,pathological features,treatment and prognosis of primary female genital tract malignant melanoma(PFGMM).Methods The data of 17 patients with PFGMM in the First Affiliated Hospital of Zhengzhou University from March 2015 to July 2023 were retrospectively analyzed.Kaplan-Meier test and log-rank test were used to analyze the relationship between related factors and prognosis.Results The median age of 17 patients was 52 years old(33-70 years).Vulva was the predominant site,followed by vagina and cervix.The main clinical manifestations were vulvar mass and irregular vaginal bleeding.All the patients received surgical treatment,and 3 of them did not receive adjuvant treatment.Median follow-up was 24 months(6-113 months).The 1-year,2-year,and 5-year progression-free survival rates were 80.5%,42.9%and 21.5%respectively,and the overall survival rates were 94.1%,63.2%and 24.6%respectively.International federation of gynecology and obste-tries stage was the main factor affecting prognosis,and patients with stage Ⅲ/Ⅳ had poorer prognosis than those with stage Ⅰ/Ⅱ(P=0.001).Conclusion PFGMM tends to occur in postmenopausal women,with a low incidence,but a high degree of malignancy and atypical clinical symptoms.Immunohistochemistry is very important for the identification of the disease.Surgical treatment combined with adjuvant therapy is the most popular clinical approach for malignant melanoma currently.Patients with late clinical stage tend to have a poor prognosis.
