Related Factors of Extracapsular Invasion of Prostate Cancer Based on Multi-parameter Magnetic Resonance Indexes and Clinical Characteristics

Abstract：Objective The related factors of extracapsular invasion of prostate cancer were analyzed based on multi-parameter magnetic resonance indexes and clinical characteristics.Methods The clinical data of 109 patients with prostate cancer who underwent radical prostatectomy from January 2022 to January 2025 in Puyang Hospital of Traditional Chinese Medicine were retrospectively analyzed.According to the postoperative pathological results,they were divided into invasive group(53 cases)and non-invasive group(56 cases).Multi-parameter MRI indexes and clinical features were compared between the two groups,and the influencing factors were analyzed by multivariate logistic regression analysis,and the regression equation was constructed.Results Compared with the non-invasive group,the invasive group had smaller prostate volume,higher maximum diameter of lesion,score of international society of urological pathology(ISUP),levels of serum total prostate-specific antigen(tPSA),prostate-specific antigen density(PSAD),prostate-specific antigen(PSA),Ktrans and kep,and lower apparent diffusion coefficient(ADC)(P＜0.05).Multivariate logistic regression analysis showed that small prostate volume,high maximum diameter of lesion,high score of ISUP,high level of serum tPSA,high level of serum PSAD,high Ktrans,high level of kep and low ADC were independent influencing factors of extracapsular invasion of prostate cancer(P＜0.05).According to the diagnostic probability logit(P),the ROC curve for predicting the extracapsular invasion of prostate cancer was drawn.When logit(P)＞0.33,the area under the curve and x2 were 0.901 and 13.445,respectively,and the 95％CI was 0.829-0.950,and the diagnostic sensitivity and specificity were 81.13％and 89.29％,respectively.Conclusion Small prostate volume,high maximum diameter of lesion,high score of ISUP,high level of serum tPSA,high level of serum PSAD,high Ktrans,high level of kep and low ADC were independent influencing factors of extracapsular invasion of prostate cancer.The regression equation constructed based on this had high predictive value.
