Clinical Efficacy Comparison of Neuroendoscopic Hematoma Evacuation and Single Hole Drilling Drainage for the Treatment of Chronic Subdural Hematoma

Abstract:Objective To compare the clinical efficacy of neuroendoscopic hematoma evacuation and single hole drilling drainage in the treatment of chronic subdural hematoma(CSDH).Methods A retrospective study was conducted to collect medical records of 33 patients with CSDH who underwent single hole drilling drainage surgery at Xuchang Hospital from September 2019 to December 2021,and included them in the control group.Medical records of 28 CSDH patients who underwent neuroendoscopic hematoma evacuation surgery at Xuchang Hospital from January 2022 to April 2024 were collected,and included in the observation group.The perioperative indexes,hematoma clearance rate and postoperative complications of the two groups were statistically analyzed.Markwalder classification was used to evaluate the neurological function of patients before operation and at discharge.The recurrence of the two groups at 3 months after operation was compared.Results The surgery time in observation group was longer than that in control group,and the hospitalization time was shorter than that in control group(P<0.05).The postoperative hematoma volume in observation group was less than that in control group,and the hematoma clearance rate was higher than that in control group(P<0.05).There was no statistical significant difference in the incidence of postoperative complications between the two groups(P>0.05).After surgery,both groups showed improvement in Markwalder grading compared to preoperative levels(P<0.05),while there was no statistical significant difference between the groups(P>0.05).Three months after surgery,the recurrence rate in observation group was lower than that in control group(P<0.05).Conclusion Both endoscopic hematoma evacuation and single hole drilling drainage can significantly improve neurological function in patients with CSDH,and the two are equally safe.However,the former has a shorter hospital stay,higher hematoma clearance rate,and lower recurrence rate.
