Correlation of Serum DCLK1,CHI3L1,LAIR2 Levels with Pathological Characteristics and Clinical Prognosis in Patients with Non-small Cell Lung Carcinoma

Abstract:Objective To investigate the correlation between the levels of serum doublecortin like kinase 1(DCLK1),chitinase 3-like protein 1(CHI3L1),and leukocyte-associated immunoglobulin-like receptor 2(LAIR2)and pathological characteristics in patients with non-small cell lung carcinoma(NSCLC),and analyze their prognostic predictive value.Methods A total of 108 NSCLC patients in the First Affiliated Hospital of Zhengzhou University from March 2021 to March 2023 were selected as the study group.During the same period,54 patients with benign lung lesions were selected as the benign lesion group,and 36 healthy volunteers were selected as the control group.The levels of DCLK1,CHI3L1,and LAIR2 in the serum of the three groups were compared.The levels of serum DCLK1,CHI3L1,and LAIR2 were compared among patients with different pathological characteristics in the study group,and their correlation with pathological characteristics was analyzed.The study group performed radical surgery for lung cancer,and divided patients into poor prognosis subgroup and good prognosis subgroup based on their prognosis one year after surgery.The levels of serum DCLK1,CHI3L1,and LAIR2 were compared before and one month after surgery.Cox regression analysis was used to analyze the factors affecting prognosis.The prognostic value of serum levels of DCLK1,CHI3L1,and LAIR2 one month after surgery was analyzed.Results The serum levels of DCLK1,CHI3L1,and LAIR2 in the study group were higher than those in the benign lesion group and the control group,and the benign lesion group was higher than the control group(P<0.05).The levels of serum DCLK1,CHI3L1,and LAIR2 in patients with TNM stage Ⅲa>those in patients with stage Ⅱ>those in patients with stage Ⅰ,and the levels of serum DCLK1,CHI3L1,and LAIR2 in patients with tumor diameter≥3 cm were higher than those in patients with tumor diameter<3 cm.The levels of serum DCLK1,CHI3L1,and LAIR2 in patients with lymph node metastasis were higher than those in patients without lymph node metastasis.The levels of serum DCLK1,CHI3L1,and LAIR2 in patients with low differentiation were higher than those in patients with medium and high differentiation(P<0.05).The levels of serum DCLK1,CHI3L1,and LAIR2 were positively correlated with TNM staging,tumor diameter,and lymph node metastasis,while negatively correlated with differentiation degree(P<0.05).The levels of serum DCLK1,CHI3L1,and LAIR2 in the subgroup with poor prognosis were higher than those in the subgroup with good prognosis one month after surgery(P<0.05).Cox regression analysis showed that lymph node metastasis and serum levels of DCLK1,CHI3L1,and LAIR2 were factors that negatively affected prognosis(P<0.05).The area under the curve(AUC)values of serum DCLK1,CHI3L1,LAIR2 levels and their combination for predicting prognosis were 0.743,0.730,0.739,and 0.936,respectively,and the AUC value of the combination prediction was greater than that of the single indicator prediction(P<0.05).Conclusion The levels of serum DCLK1,CHI3L1,and LAIR2 in NSCLC patients are elevated and closely related to clinical pathological characteristics and poor prognosis.They have certain predictive value for prognosis,and the combined detection of their levels has better predictive efficacy for prognosis.
