Abstract:Objective To investigate the relationship between the levels of serum platelet factor 4(PF4),serum amyloid protein A1(SAA1),and formyl peptide receptor 2(FPR2)and infertility due to endometriosis. Methods A total of 207 patients with endometriosis were prospectively selected at Zhengzhou People's Hospital from August 2022 to January 2024.Serum levels of PF4,SAA1,and FPR2 were measured using enzyme-linked immunosorbent assay after admission.Patients were followed up for 1 year and divided into an infertility group and a non-infertility group based on whether they were infertile. The general characteristics and serum levels of PF4,SAA1,and FPR2 were compared between the two groups.Logistic regression analysis was used to analyze the influencing factors of endometriosis infertility. The relationship between PF4,SAA1,FPR2 and infertility risk in patients with endometriosis was analyzed by restricted cubic spline analysis(RCS)model. The receiver operating characteristic(ROC)curve was drawn to evaluate the predictive value of PF4,SAA1 and FPR2 in patients with endometriosis for infertility. Results Among 207 patients with endometriosis,200 completed the follow-up.Of these,80 were infertile and were included in the infertility group, while the remaining 120 were included in the non-infertility group. The proportion of pelvic adhesion of patients in the infertility group was 17.50%,which was higher than that in the non-infertility group(7.50%),and the difference was statistically significant(P<0.05).The serum levels of PF4,SAA1,and FPR2 of patients in the infertility group were higher than those in the non-infertility group, the differences were statistically significant(P<0.05).Logistic regression analysis showed that pelvic adhesion,PF4,SA A1 and FPR2 were the influencing factors of infertility in patients with endometriosis(P<0.05).The RCS test for the linear hypothesis of serum PF4,SAA1,and FPR2 in relation to infertility due to endometriosis revealed that, after adjusting for other covariates, the levels of serum PF4,SAA1,and FPR2 exhibited a dose-response relationship with the risk of infertility in patients with endometriosis(P<0.05).When serum PF4,SAA1 and FPR2 were ≥ 22.45 μg·L-1,34.98 mg·L-1,178.44 ng·L-1,respectively,the risk of infertility in patients with endometriosis increased with the increase of their levels. The ROC curve indicated that the area under the curve(AUC)values for serum PF4,SAA1,and FPR2 in predicting infertility due to endometriosis were 0.752,0.729,and 0.788,respectively.The combined AUC value of these three indicators was 0.911,which was higher than that of each indicator alone(P<0.05).Conclusion The elevated levels of serum PF4,SAA1,and FPR2 in patients with endometriosis are closely associated with infertility. The combined detection of these three markers exhibits high predictive efficacy, providing a new strategy for early clinical identification of high-risk infertility patients and targeted intervention.
