Risk Factors and Construction of a Prediction Model for Bloodstream Infection in Patients with Severe Cerebral Hemorrhage

Abstract:Objective To explore the risk factors for bloodstream infection(BSI)in patients with severe cerebral hemorrhage and construct a predictive model.Methods A total of 120 patients with severe cerebral hemorrhage admitted to Zhengzhou Central Hospital Affiliated to Zhengzhou University from June 2022 to January 2025 were selected.The occurrence of BSI was recorded,and univariate and multivariate logistic regression analyses were conducted to identify the risk factors for BSI.A personalized prediction model was established,and the predictive value was evaluated using the receiver operating characteristic(ROC)curve.Results Out of 120 patients,30 had BSI,with an incidence rate of 25.00%.Compared with the uninfected group,the infection group had a higher proportion of patients with a history of diabetes,puncture frequency ≥ 2,and femoral vein catheterization,and a lower serum albumin level(P<0.05).Multivariate logistic regression analysis showed that history of diabetes(OR=4.908),number of punctures ≥2(OR=3.755),catheter puncture site being femoral vein(OR=2.385),and serum albumin level(OR=0.657)were independent risk factors for BSI in patients with severe cerebral hemorrhage(P<0.05).The independent risk factors were included in the model,and the ROC curve showed an area under the curve(AUC)of 0.871(95%CI:0.794-0.948),a sensitivity of 0.925,and a specificity of 0.722.The AUC of the validation set was 0.844(95%CI:0.744-0.943).The Hosmer-Lemeshow goodness-of-fit test(x2=1.148,P=0.284)indicated good model fit.Conclusion Factors influencing BSI in patients with severe cerebral hemorrhage include history of diabetes,number of punctures,catheter puncture site,and serum albumin level.Constructing a predictive model can help predict the risk of BSI and provide a reference for clinical diagnosis,treatment,and prevention.
