Clinical Analysis of 11 Cases of Ralstonia pickettii Infection

Abstract:Objective To investigate the clinical characteristics and treatment strategies of cases involving Ralstonia pickettii.Methods A retrospective study was conducted to collect and analyze the general information,underlying diseases,laboratory results,clinical manifestations,radiological findings,antimicrobial susceptibility test results,and treatment outcomes of 11 patients from the Surgical Intensive Care Unit of the First Affiliated Hospital of Zhengzhou University from January 2021 to December 2023.Results All 11 patients with Ralstonia pickettii infections had weakened immune systems,had undergone artificial airway establishment and mechanical ventilation,and had co-infections with other bacteria.CT scans revealed interstitial lung changes with pleural thickening and interlobular septal thickening in most cases,while some cases showed cystic fibrotic changes in the lungs.Antimicrobial susceptibility testing showed that Ralstonia pickettii was resistant to polymyxin and ticarcillin/clavulanate but sensitive to ciprofloxacin,doxycycline,minocycline,trimethoprim/sulfamethoxazole,and tigecycline.For carbapenems,the susceptibility results were inconsistent,with most strains resistant to meropenem but varying from sensitive to intermediate for imipenem.After treatment with antibiotics sensitive to Ralstonia pickettii,9 patients improved,and 2 died,but the cause of death was not directly due to Ralstonia pickettii infection.Conclusion As an opportunistic pathogen,Ralstonia pickettii is prone to cause multiple infections in high-risk environments such as ICUs,and the treatment outcomes are generally favorable.
