Clinical Characteristics and Prognosis of Severe Primary Sjögren's Syndrome

Abstract：Objective To investigate the clinical features and prognosis of patients with primary Sjögren's syndrome(PSS)in ICU.Methods A retrospective study of patients with PSS admitted to ICU of the First Affiliated Hospital of Zhengzhou University from January 2019 to December 2023 was conducted.Admission clinical data,including age,gender,underlying diseases and laboratory indicators,were recorded.Acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)scores and sequential organ failure assessment(SOFA)scores were also recorded.Univariate analysis and multivariate logistic regression were used to analyze the effects of each index on the prognosis of patients.The predictive value of APACHE Ⅱ score for in-hospital mortality risk was analyzed using receiver operating characteristic(ROC)curve.Results Of 45 patients with PSS in ICU,8 died,with a fatality rate of 17.78％.Univariate analysis showed statistically significant differences in length of ICU stay,other infections,cardiovascular dysfunction,central nervous dysfunction,APACHE Ⅱ score,SOFA score,septic shock,albumin,prothrombin time activity,lactate dehydrogenase,procalcitonin elevation fold,prothrombin time,international normalized ratio,and lactate between the two groups(P＜0.05).Multivariate logistic regression analysis showed that APACHE Ⅱ score(P=0.006,OR=1.227,95％CI:1.059-1.421)was an independent risk factor for in-hospital death in ICU PSS patients.ROC curve analysis showed that the area under the curve(AUC)of APACHE Ⅱ score for predicting in-hospital death risk in ICU PSS patients was 0.94.When the cut-off value of APACHE Ⅱ score was 20.5,the sensitivity was 87.5％and the specificity was 94.6％.Conclusion APACHE Ⅱ score has a certain predictive value for in-hospital death risk in PSS patients in ICU.
