Correlation Between OCT,OCTA Indicators and Visual Function Recovery in Patients with High Myopia and Macular Hole Associated Retinal Detachment

Abstract:Objective To investigate the correlation between preoperative optical coherence tomography(OCT)and optical coherence tomography angiography(OCTA)indicators and visual function recovery in patients with high myopia and macular hole associated retinal detachment(MHRD).Methods A total of 143 patients with high myopia and MHRD who underwent vitrectomy combined with internal limiting membrane filling surgery in Zhengzhou People's Hospital from June 2023 to January 2025 were included.According to the improvement in best corrected visual acuity(BCVA)at 6 months postoperatively compared with preoperatively,they were divided into a good prognosis group(90 cases,with BCVA improvement ≥0.2 logMAR)and a poor prognosis group(53 cases,with BCVA improvement<0.2 logMAR).Preoperative central macular thickness(CMT)was measured by OCT,and preoperative choriocapillary blood flow area(CBFA)and diameter of ellipsoid zone absence(DEZA)were measured by OCTA.General data and levels of CMT,CBFA,and DEZA were compared between the two groups.Spearman correlation analysis was used to analyze the correlation between visual function recovery and CMT,CBFA,and DEZA.Binary logistic regression analysis was used to identify independent risk factors for postoperative visual function recovery.Results The poor prognosis group had larger aperture diameter,higher preoperative BCVA(logMAR),CMT,and DEZA levels,and lower CBFA level than the good prognosis group(P<0.05).Poor visual function prognosis in patients with high myopia and MHRD was positively correlated with preoperative CMT and DEZA levels,and negatively correlated with CBFA level(P<0.05).Binary logistic regression analysis showed that larger aperture diameter,higher preoperative BCVA(logMAR),CMT,and DEZA levels,and lower CBFA level were independent risk factors for poor postoperative visual function prognosis in patients with high myopia and MHRD(P<0.05).Conclusion Higher preoperative CMT and DEZA levels and lower CBFA level are independent risk factors for poor postoperative visual function prognosis.Monitoring preoperative CMT,CBFA,and DEZA levels can help understand macular microstructural changes and assist in evaluating postoperative visual function recovery.
