Effect of Modified Surgical Method on Hemodynamics and Coagulation Function in Patients Undergoing Veno-Arterial Extracorporeal Membrane Oxygenation Withdrawal and Its Safety Evaluation

Abstract:Objective To investigate the effect of modified surgical method on hemodynamics and coagulation function in patients undergoing veno-arterial extracorporeal membrane oxygenation(VA-ECMO)withdrawal,and to evaluate its safety.Methods This retrospective cohort study included 56 patients who underwent VA-ECMO and were successfully weaned in stable condition in Henan No.3 Provincial People's Hospital from January 2020 to December 2023.According to the surgical method of VA-ECMO withdrawal,the patients were divided into the traditional surgery group(30 cases)and the modified surgery group(26 cases).The differences in weaning time,bleeding volume,hemodynamics,coagulation function and postoperative complications were compared between the two groups.Results Compared with the traditional surgery group,the modified surgery group had significantly shorter VA-ECMO weaning time[(84.53±7.82)min vs(137.94±13.00)min,P<0.001]and significantly less intraoperative bleeding volume[(30.70±5.38)mL vs(75.93±9.16)mL,P<0.001].There were no significant differences in the levels of heart rate(HR),mean arterial pressure(MAP),central venous pressure(CVP)and cardiac output(CO)between the modified surgery group and the traditional surgery group(P>0.05).The levels of prothrombin time(PT)and activated partial thromboplastin time(APTT)in the modified surgery group were significantly higher than those in the traditional surgery group(P<0.05),while the levels of fibrinogen(FIB)and thrombin time(TT)were significantly lower than those in the traditional surgery group(P<0.05).The overall incidence of postoperative complications in the modified surgery group was lower than that in the traditional surgery group(15.38%vs 46.67%,P=0.021).Conclusion The modified surgical method can reduce the weaning time and postoperative bleeding volume of VA-ECMO withdrawal,improve the coagulation function,and reduce the risk of postoperative complications.It is a safe and effective VA-ECMO withdrawal method,which is worthy of clinical promotion and use.
