Characteristics of Cytokine Gene Polymorphism and Other Indicators in Patients with Chronic Obstructive Pulmonary Disease Complicated by Pulmonary Fungal Infection

Abstract:Objective To explore the characteristics of cytokine gene polymorphism,lymphocyte and blood routine indicators in patients with chronic obstructive pulmonary disease(COPD)complicated with pulmonary fungal infection.Methods A total of 52 patients with COPD complicated with pulmonary fungal infection treated in Henan Provincial Third People's Hospital from January 2021 to September 2024 were selected as the observation group,while 57 patients with simple COPD were selected as the control group.The distribution of fungi was analyzed,and the differences in cytokine gene polymorphism,lymphocyte and blood routine indicators between the observation group and the control group were compared.Results A total of 63 strains of fungi were detected in the observation group,with Candida albicans and Aspergillus fumigatus being the main ones,accounting for 38.10%and 20.63%,respectively.There was no statistically significant difference in genotype between the observation group and the control group at the interleukin-1β(IL-1β)gene locus rs3136558 and rs1 143627,and the tumor necrosis factor-α(TNF-α)gene locus rs1799724(P>0.05).There was a statistically significant difference in genotype between the observation group and the control group at the interleukin-12(IL-12)gene locus rs2243115 and rs3212227,as well as the TNF-α gene locus rs1800629(P<0.05).The CD3+CD4+T lymphocytes and CD4+/CD8+ratio in the observation group were(33.95±5.03)%and(1.22±0.33),respectively,which were lower than those in the control group(P<0.05).While the CD3+CD8+T lymphocytes were(27.80±6.69)%,which was higher than those in the control group(P<0.05).There was no statistically significant difference between the two groups of blood routine indicators,including white blood cell count,monocyte count,eosinophil count,neutrophil count,red blood cell count,hemoglobin,hematocrit,and mean platelet volume(P>0.05).The platelet count of the observation group was lower than that of the control group(P<0.05),while the red blood cell distribution width was higher than that of the control group(P<0.05).Conclusion Compared with patients with simple COPD,there are significant differences in IL-12 and TNF-α gene polymorphisms,T lymphocytes,blood routine indicators platelet and red blood cell distribution width in patients with COPD complicated with pulmonary fungal infection,which may play an important role in the process of pulmonary fungal infection.
