Impact of Non-pregnancy Laparoscopic Cervical Cerclage and Pregnancy Transvaginal Cervical Cerclage on Delivery Outcomes

Abstract:Objective To analyze the effect of non-pregnancy laparoscopic cervical cerclage and pregnancy transvaginal cervical cerclage on the prevention of premature delivery and its influence on maternal and infant outcomes based on propensity score matching(PSM).Methods A retrospective study design was used in this study to collect the clinical data of patients with cervical insufficiency(CIC)admitted to the 988 th Hospital of the Joint Logistics Support Force from January 2020 to December 2023.According to the treatment plan,70 patients who underwent pregnancy transvaginal cervical cerclage were set as the pregnancy transvaginal group,and 80 patients who underwent non-pregnancy laparoscopic cervical cerclage were set as the non-pregnancy laparoscopic group.The PSM nearest neighbor matching method was used to match the general data of the two groups by 1∶1,and the baseline confounding factors of the two groups were controlled.After matching,50 patients in each group were successfully matched.The perioperative related indicators,pregnancy outcomes,neonatal outcomes,and complications were compared between the two groups.Results The intraoperative blood loss in the non-pregnancy laparoscopic group was less than the pregnancy transvaginal group,and hospitalization time was shorter than the pregnancy transvaginal group(P<0.05).There was no significant difference in the operation time between two groups(P>0.05).The gestational age in the non-pregnancy laparoscopic group was longer than that in the pregnancy transvaginal group,the full-term delivery rate(40.00%)was higher than that(18.00%)in the pregnancy transvaginal group,the cesarean section rate(92.00%)was higher than that(6.00%)in the pregnancy transvaginal group,the neonatal birth weight was higher than that in the pregnancy transvaginal group(P<0.05).There were no significant differences in the incidence of vaginal infection,amniotic sac infection,cerclage band shedding or displacement,hematoma formation,threatened abortion,placenta previa,premature rupture of membranes,fetal distress,oligohydramnios,fetal survival rate and neonatal transfer rate to NICU between the two groups(P>0.05).Conclusion Compared with pregnancy transvaginal cervical cerclage,non-pregnancy laparoscopic cervical cerclage has less intraoperative bleeding,shorter hospital stay,and a higher rate of full-term pregnancy delivery,which is beneficial for improving maternal and infant prognosis.
