Effect of Video-Assisted Thoracoscopic Surgery Pulmonary Bullectomy Combined with Pleural Fixation in the Treatment of Spontaneous Pneumothorax in the Elderly

Abstract：Objective To analyze the therapeutic effect of video-assisted thoracoscopic surgery(VATS)pulmonary bullectomy combined with pleural fixation in the treatment of spontaneous pneumothorax in the elderly.Methods Retrospective analysis was conducted on the medical records of 80 elderly patients with spontaneous pneumothorax from Henan Chest Hospital from April 2021 to February 2023,according to the surgical plan,patients who underwent VATS pulmonary bullectomy were divided into a control group(40 cases)and an observation group(40 cases).The clinical efficacy,surgical and postoperative conditions,pain status[visual analog scale(VAS)],oxidative stress response[superoxide dismutase(SOD),malondialdehyde(MDA)],the incidence of complications and the recurrence rate of the disease were compared between the two groups.Results Compared with the control group,there was no significant difference in the total surgical effectiveness rate and complication rate between the observation group and the control group(P＞0.05).When comparing the two groups,the surgical and postoperative indicators of the observation group were better(P＜0.05).The interaction effect of VAS score between groups,time,and interaction was statistically significant(P＜0.05).Before the operation,there was no statistically significant difference in the levels of SOD and MDA between the two groups(P＞0.05).After the operation,the levels of all indicators in both groups improved,and the observation group was better(P＜0.05).When comparing the two groups,the recurrence rate of the observation group at 3,6,and 12 months after the operation was lower(P＜0.05).Conclusion The combination of VATS bullectomy and pleurodesis for the treatment of elderly patients with spontaneous pneumothorax is effective,it can help patients recover more quickly,cause less postoperative pain,reduce the body's stress response,have fewer complications,and have a lower recurrence rate of the disease.
