Effect of Ultrasound-Guided Transversus Abdominis Plane Block on Recovery Quality and Cardiovascular Stress in Patients Undergoing Laparoscopic Hysterectomy

Abstract:Objective To explore the effects of ultrasound-guided transversus abdominis plane(TAP)block on recovery quality and cardiovascular stress.Methods A total of 60 patients undergoing laparoscopic hysterectomy at Luoyang First People's Hospital from December 2021 to December 2023 were enrolled.They were divided into group A(general anesthesia group),group B(general anesthesia and lateral approach group),and group C(general anesthesia and posterior approach group),with 20 cases in each group.General anesthesia was administered to all three groups after tracheal intubation under general anesthesia and before the start of the surgery.Group B underwent lateral approach ultrasound-guided TAP block,while patients in group C underwent posterior approach ultrasound-guided TAP block.The sedation efficacy[observer's assessment of alertness/sedation scale(OAA/S)],analgesic efficacy[visual analogue scale(VAS)],anesthetic consumption,cardiovascular indicators(heart rate and systolic blood pressure),and adverse reactions were compared at different time points post operatively.Results The OAA/S sedation scores and VAS of group C at 2 hours,6 hours,12 hours and 24 hours after the operation were all lower(P<0.05).The usage of various anesthetic drugs at each time point in group C was less(P<0.05).One minute after skin incision,the heart rate and systolic blood pressure levels of patients in each group were higher than those before skin incision,the heart rate and systolic blood pressure levels in group C were both lower(P<0.05).The incidence of nausea,vomiting and hypotension in group C was lower(P<0.05).Conclusion Posterior approach ultrasound-guided TAP block can improve the sedative and analgesic effects of patients undergoing laparoscopic hysterectomy,while reducing the dosage of anesthetic drugs during and after the operation,and lowering cardiovascular stress and adverse reactions.
