Abstract:Objective To understand the distribution and changes of multidrug resistant organisms in a Grade A tertiary Chinese medicine hospital from 2018 to 2024,and grasp the epidemic trend of multidrug resistant organisms, and provide reference for the prevention and control strategies of multidrug resistant organisms. Methods Multidrug resistant organisms in a Grade A tertiary Chinese medicine hospital from 2018 to 2024 were selected to carry out target surveillance, and the detection situation, specimen type, department distribution and infection situation of key target bacteria were statistically analyzed. Results From 2018 to 2024,the detection rate of multidrug resistant organisms showed an overall decreasing trend(x2=386.328,P<0.001),among which carbapenem-resistant Acinetobacter baumannii(CR-AB)had the highest detection rate,methicillin-resistant Staphylococcus aureus(MRSA)and carbapenem-resistant Enterobacteriaceae(CRE)increased year by year.Incidence of nosocomial infection of multidrug resistant organisms in CRE was the highest, and it increased year by year(x2=66.747,P<0.001).Klebsiella pneumoniae accounted for the largest proportion of bacteria isolated from CRE.The multidrug resistant organisms were mainly sputum, followed by blood, the proportion of alveolar lavage fluid increased year by year(x2=364.268,P<0.001).The top four departments with multidrug-resistant organisms distribution were intensive care unit, respiratory department, encephalopathy department, and neurosurgery department. Conclusion MRSA and CRE are the focus of the next targeted monitoring and prevention and control management of the Chinese medicine hospital. At the same time,it is necessary to continue to strengthen the inspection of sterile specimens, implement multidrug resistant organisms prevention and control measures in key departments such as intensive care unit and respiratory department, and reduce the nosocomial infection of multidrug resistant organisms.
