Relationship Between Blood Glucose Variability Index and Complications as Well as Joint Function Recovery in Elderly Patients Undergoing Knee Joint Surface Replacement Surgery

Abstract:Objective To explore the relationship between blood glucose variability index and complications,as well as joint function recovery in elderly patients undergoing knee arthroplasty surgery.Methods A retrospective analysis was conducted on 47 elderly patients who underwent total knee surface replacement without complications in Pingdingshan First People's Hospital from June 2023 to November 2024 as the no-complication group.Additionally,33 patients who developed complications were included as the complication group.Six months postoperatively,the Western Ontario and McMaster Universities score was used and the patients were divided into a good recovery group(WOMAC score ≤ 35 points,51 cases)and a poor recovery group(WOMAC score>35 points,29 cases)according to their joint recovery status.The preoperative hemoglobin(Hb)concentration,albumin(ALB)concentration,red blood cell(RBC)count,white blood cell(WBC)count,C-reactive protein(CRP),interleukin-6(IL-6)and intraoperative blood glucose variability index level with patients with different complications and different recovery conditions were compared.Multivariate logistic regression was used to analyze the influencing factors of complications and poor recovery in elderly patients undergoing knee surface replacement.Results There was no statistically significant difference in Hb,ALB,RBC,and WBC among patients with different complications(P>0.05),the CRP,IL-6,and blood glucose variability index of the patients in the complications group were higher than the no-complication group(P<0.05).Logistic regression analysis found that CRP,IL-6 and blood glucose variability index were all influencing factors of complications of knee joint surface replacement in elderly patients(P<0.05).There is no statistical difference in Hb,ALB,RBC,and WBC between patients with different joint function recovery conditions(P>0.05).However,the CRP,IL-6,and blood glucose variability index of patients in the poor recovery group were significantly higher than Good recovery group(P<0.05).Logistic regression analysis found that CRP,IL-6 and blood glucose variability index were all influencing factors for poor postoperative recovery in elderly patients after knee surface replacement surgery(P<0.05).Conclusion The expressions of CRP,IL-6 and blood glucose variability index are factors affecting postoperative complications and poor recovery in elderly patients undergoing knee surface replacement.Therefore,controlling blood sugar levels,monitoring and regulating IL-6 and CRP levels are of great significance for reducing postoperative complications and promoting joint function recovery in elderly patients undergoing knee surface replacement.
