Value of 3D-ASL Cerebrovascular Perfusion Parameters and miR-720 in Predicting Postoperative Recurrence of Glioma

Abstract:Objective To investigate the cerebral perfusion parameters of magnetic resonance three-dimensional arterial spin labeling(3D-ASL)and the expression level of microRNA-720(miR-720)in patients with glioma,and to analyze its incremental value in predicting postoperative recurrence.Methods A total of 258 patients with glioma admitted to General Hospital of Pingmei Shenma Group from January 2022 to January 2024 were prospectively selected as the research objects.According to the recurrence at 1 year after operation,they were divided into recurrence group and non-recurrence group.The preoperative 3D-ASL cerebrovascular perfusion parameters[relative blood flow(rCBF),relative blood volume(rCBV)]and the expression level of miR-720 in peripheral blood mononuclear cells were compared between the two groups.Logistic regression equation was used to analyze the influencing factors of postoperative recurrence.Restricted cubic spline model(RCS)was used to analyze the relationship between 3D-ASL cerebrovascular perfusion parameters,miR-720 and postoperative recurrence.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of 3D-ASL cerebrovascular perfusion parameters and miR-720 for postoperative recurrence.Results The follow-up rate was 97.67%(252/258),and the recurrence rate within 1 year after operation was 62.69%(158/252).The platelet count(PLT)and plateletcrit(PCT)in the recurrent group were higher than those in the non-recurrent group,while the mean platelet volume(MPV)was lower than that in the non-recurrent group(P<0.05).The expression levels of miR-720 in rCBF,rCBV and peripheral blood mononuclear cells in the recurrence group were higher than those in the non-recurrence group,and the differences were statistically significant(P<0.05).PLT,PCT,MPV,rCBF,rCBV and miR-720 were the influencing factors of postoperative recurrence(P<0.05).There was a non-linear positive correlation between rCBF,rCBV,miR-720 and postoperative recurrence of glioma(P<0.05).When rCBF,rCBV and miR-720 were lower than 2.80,8.00 and 3.85,the risk curve of postoperative recurrence tended to be stable,when rCBF,rCBV and miR-720 were higher than 2.80,8.00 and 3.85,the risk of postoperative recurrence increased.Model A was established by PLT,PCT,MPV,rCBF,rCBV and miR-720,and model B was established by rCBF,rCBV and miR-720,the AUC of model A in predicting postoperative recurrence was higher than that of model B and the AUC of each index alone,and the difference was statistically significant(P<0.05).Conclusion The expression levels of rCBF,rCBV and miR-720 in peripheral blood mononuclear cells of patients with postoperative recurrence of glioma are significantly higher than those of non-recurrent patients,and there is a nonlinear positive correlation between rCBF,rCBV,miR-720 and postoperative recurrence.Combined detection of their levels has incremental value in predicting postoperative recurrence.
