Application of Modified POSSUM Scoring System in the Evaluation of Intertrochanteric Fractures

Abstract:Objective To explore the value of improved physiology and surgical severity scoring system(POSSUM)in the postoperative prognosis evaluation of intertrochanteric fracture.Methods A total of 80 patients who underwent surgical treatment for intertrochanteric femoral fractures at the Third People's Hospital of Henan Province from February 2020 to February 2023 were selected for a 1-year follow-up.Relevant clinical data were collected according to the modified POSSUM scoring system,and the complications and deaths were predicted during the follow-up period,and compared with the actual complications and deaths.Patients with complications and death were included in the poor prognosis group,and other patients without complications and death were included in the good prognosis group.The clinical data between the groups were compared,and their modified POSSUM score was calculated to explore their value of predicting the poor prognosis of patients.Results There was no statistically significant difference between the predicted complications and deaths of the 80 patients based on the modified POSSUM scoring system and the actual occurrences(P>0.05).There was no statistically significant difference in hemoglobin,leukocyte count,albumin,operation time,intraoperative bleeding,gender,surgical time,anesthesia and internal fixation between the poor and good prognosis groups(P>0.05).The age,and the proportion of blood urea nitrogen and electrocardiagram abnormalities were higher than the good prognosis group(P<0.05).Based on the collected clinical data,the modified POSSUM score and surgical severity score of the patients were calculated.The modified POSSUM score and surgical severity score of the poor prognosis group were higher than those of the good prognosis group(P<0.05).After the receiver operating characteristic(ROC)curve analysis,the area under the curve(AUC)of modified POSSUM score was 0.834,95%CI was 0.704-0.964,the optimal cut-off value was 25.875 points,a sensitivity of 0.810,a specificity ratio of 0.847.Conclusion Modified POSSUM has certain value in postoperative prognosis evaluation of intertrochanteric fracture,is relatively accurate for prediction of complications and death,and can assist clinical evaluation of patient prognosis after intertrochanteric fracture.
