Correlation Between CRP/ALB Ratio and Clinical Outcomes in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease

Abstract:Objective To investigate the relationship between C-reactive protein(CRP)/albumin(ALB)ratio and clinical outcomes in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Data were collected from 80 AECOPD patients treated at Zhengzhou TCM Hospital from January 2023 to June 2024,all of whom received non-invasive positive pressure ventilation treatment,and the serum CRP/ALB ratio was measured before treatment.The outcome of weaning within 28 days was used as the evaluation index of clinical outcome,and the influencing factors of weaning outcome in AECOPD patients were analyzed.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of CRP/ALB ratio for weaning outcome.Results In the 80 patients,62 patients successfully weaned,with a weaning success rate of approximately 77.50%.The remaining 18 patients did not pass the weaning test and needed to continue with ventilation treatment at their original values.The weaning failure rate was 22.50%.The data of patients with successful and failed weaning were compared and the logistic regression model was constructed.The results showed that the global initiative for chronic obstructive pulmonary disease(GOLD)level 3,mechanical ventilation time>7 d,low oxygenation index,CRP/ALB ratio,and increased interleukin(IL)-6 were risk factors for weaning failure in AECOPD patients(P<0.05).The ROC curve was plotted,and the AUC value for predicting weaning outcomes in AECOPD patients using serum CRP/ALB ratio was 0.781,with an optimal cutoff value of 88.210%,a sensitivity of 0.722,and a specificity of 0.726.Conclusion The failure of weaning in AECOPD patients is related to an increase in serum CRP/ALB ratio.Testing the CRP/ALB ratio before non-invasive positive pressure ventilation treatment can help evaluate the success rate of weaning and identify high-risk populations for weaning failure.
