Relationship Between Prenatal Antibody Titer Monitoring in Pregnant Women and Hemolytic Disease of Newborn

Abstract:Objective To analyze the correlation between prenatal antibody titer monitoring in pregnant women and hemolytic disease of newborn(HDN).Methods A retrospective study was conducted to collect clinical data of 268 pregnant women with type O pregnancy who underwent prenatal examination and delivery at the Fifth Affiliated Hospital of Zhengzhou University from June 2020 to June 2024.The pregnant women were monitored for serum IgG anti-A(B)antibody titers one week before delivery,and their newborn blood types were detected after delivery.HDN testing was performed on newborns with type A or B,and the correlation between prenatal antibody titers monitoring and postpartum HDN incidence was analyzed.At the same time,the receiver operating characteristic(ROC)curve was drawn to explore the predictive effect of prenatal antibody titer monitoring on the occurrence of HDN.Results Among the 268 pregnant women with type O pregnancy,18 were matched between mother and infant,and their newborns were all positive for type O Rh(D);250 cases of maternal infant incompatibility,including 117 cases of neonatal type A and 133 cases of type B.The incidence rate of HDN was 24.80%(62/250),the incidence rate of HDN in type A was 21.37%(25/117),and the incidence rate of HDN in type B was 27.82%(37/133).There was no statistical significant difference in the incidence rate of HDN in type A and the incidence rate of HDN in type B(P>0.05).There was a statistical significant difference in the number of maternal pregnancies between the occurrence group and the non occurrence group(P<0.05).There was no statistical significant difference in other data between the two groups(P>0.05).After x2 test with Cramer's V coefficient,the results showed that the IgG anti-A(B)antibody titer of pregnant women was positively correlated with the occurrence of HDN(Cramer's V value=0.551,P<0.05).The ROC curve was plotted,and the results showed that the antibody titer predicted the area under the curve(AUC)was 0.846 for HDN occurrence,which had good predictive value.Conclusion The prenatal antibody titer of pregnant women is positively correlated with the occurrence of postpartum HDN.The higher the antibody titer,the higher the incidence of HDN,and prenatal antibody titer monitoring has good predictive value in the occurrence of HDN.
