Application Effects of Peroral Endoscopic Cardial Constriction in Gastroesophageal Reflux Disease Combined with Sleep Disorders

Abstract:Objective To explore the clinical efficacy and safety of peroral endoscopic cardial constriction in the treatment of gastroesophageal reflux disease(GERD),as well as the improvement of sleep disorders before and after treatment.Methods A randomized controlled study was conducted by enrolling 160 patients diagnosed with GERD accompanied by sleep disorders through endoscopy in Fuwai Central China Cardiovascular Hospital from March 2022 to September 2023.These patients were divided into an experimental group and a control group.The control group was treated with proton pump inhibitors(PPI)for 4 weeks,while the experimental group underwent PECC followed by PPI treatment for 4 weeks.The gastrointestinal symptoms,quality of life,and sleep conditions of the two groups of patients were compared before treatment and 3 months after treatment through the scores of the gastroesophageal reflux disease questionnaire(GERD-Q),the health related quality of life scale for GERD(GERD-HRQL),and the sleep dysfunction rating scale(SDRS).Moreover,the therapeutic effects of the two groups were compared by the total effective rate of the patients 6 months after treatment.Results There were no statistically significant differences in gender composition,average age,average disease duration,Los Angeles classification,smoking history,drinking history,anxiety and depression history,hypertension history,and pre-treatment GERD-Q,GERD-HRQL,and SDRS scores between the two groups(P>0.05).Three months after treatment,the GERD-Q and GERD-HRQL scores in the control group decreased compared with those before treatment(P<0.05),while the GERD-Q,GERD-HRQL,and SDRS scores in the experimental group all decreased compared with those before treatment(P<0.05).Three months after treatment,the GERD-Q,GERD-HRQL,and SDRS scores in the experimental group were all lower than those in the control group(P<0.05).Six months after treatment,the overall treatment effectiveness in the experimental group was 96.25%,which was higher than the 78.75%in the control group(P<0.05).No serious treatment-related complications or deaths occurred in either group of patients.Conclusion GERD patients have varying degrees of sleep disorders.Peroral endoscopic cardial constriction is safe and effective in treating GERD with a definite improvement in sleep disorders,and its advantages are more obvious compared with simple drug treatment.
