Influence of Helicobacter pylori Infection on the Gastric Function and Immune Cell Subsets of Patients with Precancerous Lesions of Gastric Cancer

Abstract:Objective To explore the influence of Helicobacter pylori infection on the gastric function and immune cell subsets of patients with precancerous lesions of gastric cancer.Methods From May 2023 to February 2025,96 patients with precancerous lesions of gastric cancer accompanied by Helicobacter pylori infection admitted to the Fifth Affiliated Hospital of Zhengzhou University were selected as the observation group,and 96 healthy individuals undergoing physical examinations during the same period were selected as the control group.Serum samples of the patients were collected,and three tests of gastric function were performed by enzyme-linked immunosorbent assay(ELISA).The levels of pepsinogen Ⅰ(PG Ⅰ),pepsinogen Ⅱ(PG Ⅱ)and gastrin-17(G-17)were determined.The levels of T lymphocyte subsets and natural killer(NK)cells were determined by immunofluorescence.The three gastric function items and the levels of T lymphocyte subsets and NK cells were compared between the two groups.The levels of T lymphocyte subsets and NK cells in the observation group were stratified.The levels of PG Ⅰ,PG Ⅱ and G-17 under different stratifications were compared,and Pearson analysis was performed to analyze the correlations between the three gastric function items and T lymphocyte subsets and NK cells.Results The level of PG Ⅰ in the observation group was lower than that in the control group,while the levels of PG Ⅱ and G-17 were both higher than those in the control group(P<0.05).The levels of CD3+,CD4+,CD4+/CD8+,and NK cells in the observation group were all lower than those in the control group,and the level of CD8+was higher than that in the control group(P<0.05).The level of PG Ⅰin the CD3+decreased group was lower than that in the CD3+normal group,while the levels of PG Ⅱ and G-17 were higher than those in the CD3+normal group(P<0.05).The level of PG Ⅰ in the CD4+decreased group was lower than that in the CD4+normal group,and the levels of PG Ⅱ and G-17 were higher than those in the CD4+normal group(P<0.05).The level of PG Ⅰ in the CD8+increase group was lower than that in the CD8+normal group,while the levels of PG Ⅱ and G-17 were higher than those in the CD8+normal group(P<0.05).The level of PG Ⅰ in the decreased CD4+/CD8+group was lower than that in the normal CD4+/CD8+group,and the levels of PG Ⅱ and G-17 were higher than those in the normal CD4+/CD8+group(P<0.05).The level of PG Ⅰ in the decreased NK cell group was lower than that in the normal NK cell group,while the levels of PG Ⅱ and G-17 were higher than those in the normal NK cell group(P<0.05).The results of Pearson analysis show that:CD3+,CD4+,CD4+/CD8+and NK cells were slightly to moderately positively correlated with PG Ⅰ(r>0),and slightly to moderately negatively correlated with PG Ⅱ and G-17,respectively(r<0).CD8+was slightly negatively correlated with PG Ⅰ(r=-0.308),and had no correlation with PG Ⅱ and G-17 respectively(the r values were 0.238 and 0.273).Conclusion Patients with precancerous lesions of gastric cancer accompanied by Helicobacter pylori infection may present with abnormal gastric mucosal function and status,and are closely related to immune function.
