Application of Regional Hepatic Blood Flow Occlusion Technique in Laparoscopic Hepatectomy for Primary Liver Cancer

Abstract:Objective To observe the effect of regional hepatic blood flow obstruction technique in patients with primary liver cancer(PLC)undergoing laparoscopic hepatectomy(LH).Methods The clinical data of 60 patients with PLC treated with LH in the Fifth Clinical Medical College of Henan University of Chinese Medicine,People's Hospital of Zhengzhou from January 2020 to January 2024 were retrospectively collected.Thirty patients who underwent intermittent total hepatic blood flow obstruction technique were included in the control group,and 30 patients who underwent regional hepatic blood flow obstruction technique were included in the observation group.The clinical indexes,liver function,hemodynamic indexes and complications before and 1 d after operation were compared between the two groups.Results Compared with the control group,the observation group had shorter surgical time and hepatic blood flow obstruction time,and less intraoperative bleeding(P<0.05).There was no statistical significant difference in hospitalization time between groups(P>0.05).The levels of aspartate aminotransferase(AST),alanine aminotransferase(ALT),total bilirubin(TBIL),and direct bilirubin(DBIL)in the observation group on the 1 st day after surgery[(41.21±3.02)U·L-1,(57.62±6.03)U·L-1,(17.06±1.95)μmol·L-1,(8.21±1.03)μmol·L-1]were lower than those in the control group[(47.51±4.62)U·L-1,(63.51±2.66)U·L-1,(19.66±2.03)μmol·L-1,(10.96±0.97)μmol·L-1](P<0.05);the albumin(ALB)level in the observation group[(34.21±2.22)g·L-1]was higher than that in the control group[(31.62±3.03)g·L-1](P<0.05).On postoperative day 1,there were no statistically significant differences in heart rate(HR),mean arterial pressure(MAP)and portal vein blood flow velocity(PVV)between the two groups(P>0.05).There was no statistical significant difference in the total incidence of postoperative complications between the groups(P>0.05).Conclusion Regional hepatic blood flow occlusion technique is beneficial for shortening operation time,reducing intraoperative blood loss,and alleviating liver function damage.
