Influencing Factors of Dural Tear in Patients with Degenerative Lumbar Diseases During Lumbar Interbody Fusion Surgery

Abstract:Objective To explore the influencing factors of dural tear(DT)during tlumbar interbody fusion surgery for patients with degenerative lumbar diseases through intervertebral foramen.Methods A retrospective study was conducted to collect clinical data of 31 patients with lumbar degenerative diseases who developed DT during lumbar interbody fusion surgery treatment at Zhengzhou Central Hospital Affiliated to Zhengzhou University from June 2020 to September 2024,as the DT group.The clinical data of 124 patients with lumbar degenerative diseases without DT were collected in the same period as the non-DT group.Univariate and multivariate logistic regression analysis were used to analyze the influencing factors of DT in patients with lumbar degenerative diseases.A nomogram prediction model was constructed based on the logistic regression analysis results.Results There were statistical significant differences between the two groups of patients in terms of age,surgical stage,gender,surgical time,combined ossification of the posterior longitudinal ligament,body mass index(BMI),and surgical approach(P<0.05).The use of multiple logistic regression analysis showed that age,surgical segment,gender,surgical time,combined ossification of the posterior longitudinal ligament,BMI,and surgical approach were the influencing factors for the occurrence of DT in patients during surgery(P<0.05).Based on the above factors,a nomogram model was constructed to predict the occurrence of DT.The internal validation method of Bootstrap was used to verify the discriminative power of the nomogram model.The results showed that the C-index of the model was 0.972,and its calibration curve had a good fit with the standard curve.The receiver operating characteristic(ROC)curve was plotted,and the area under the curve(AUC)was 0.972.Conclusion The influencing factors of DT occurrence in patients with lumbar degenerative diseases undergoing lumbar interbody fusion surgery treatment are age,surgical segment,gender,surgical time,combined ossification of the posterior longitudinal ligament,BMI,and surgical approach.The nomogram model constructed based on these influencing factors has good predictive value for DT occurrence in lumbar interbody fusion surgery treatment,which is helpful for clinical development of targeted prevention plans.
