Autologous Platelet-Rich Plasma in Patients With Low-Output Enterocutaneous Fistula

Abstract：Objective To investigate the clinical effect of autologous platelet-rich plasma(PRP)on fistula healing in patients with low-output enterocutaneous fistula.Methods A retrospective cohort study was conducted.Clinical data of 48 patients with low-output tubular enterocutaneous fistula treated in the Department of Gastrointestinal Surgery of a tertiary hospital in Henan Province from January 2021 to April 2023 were selected.According to the sealing method,they were divided into two groups,25 patients received autologous PRP sealing(PRP group)and 23 patients received fibrin glue sealing(fibrin group).The success rate of fistula closure,fistula closure time,number of sealing procedures and pain score during sealing treatment were compared between the two groups.Results A total of 48 patients were finally included in this study,including 30 males(62.5％)and 18 females(37.5％),with a mean age of(55.50±16.64)years.There were no significant differences in baseline data such as gender,age,hemoglobin,C-reactive protein,history of diabetes,location,fistula length,fistula diameter and fistula output between the PRP group and the fibrin group(P＞0.05).The success rate of fistula closure in the PRP group was higher than that in the fibrin group(88.00％vs 60.87％,P=0.030),the fistula closure time was shorter than that in the fibrin group[(9.48±3.62)days vs(14.39±5.35)days,P=0.001],and the number of sealing procedures was less than that in the fibrin group(P=0.018).The pain score in the PRP group was lower than that in the fibrin group[(0.53±0.31)points vs(0.86±0.47)points],but the difference was not statistically significant(P=0.072).Conclusion Autologous PRP for sealing low-output tubular fistula can effectively promote fistula healing,shorten hospital stay and reduce treatment costs,which is worthy of clinical application.
