Current Status and Influencing Factors of Demand for Hospice Care Among Advanced Patients with Chest Obstruction

Abstract:Objective To investigate the current level of demand for hospice care among advanced patients with chest obstruction and analyze the relevant influencing factors.Methods From May 2024 to March 2025,186 advanced patients with chest obstruction in the Department of Geriatrics of the the First Affiliated Hospital of Henan University of Traditional Chinese Medicine were selected as the subjects of the survey.Questionnaire surveys were conducted using general information questionnaires,palliative care needs screening tool(PCST),family APGAR questionnaire(APGAR),brief illness perception questionnaire(BIPQ),depression-anxiety-stress self-assessment scale(DASS),and Fried physical phenotype(FPP).Differences in the degree of palliative care needs among patients with different clinical characteristics were compared.Multiple linear regression analysis was used to analyze the factors affecting the degree of palliative care needs among advanced patients with chest obstruction.Furthermore,importance matrix analysis was employed to analyze the distribution areas of improvement measures for each factor.Results Ultimately,177 advanced patients with chest obstruction entered the study.The total PCST score was(31.16±4.60)scores,with the highest score in physical symptoms and the lowest score in care goals.The PCST score for advanced patients aged<80 years was(27.64±4.32)scores,significantly lower than that of patients aged ≥ 80 years[(36.29±3.26)scores],the PCST score for patients with self-care ability was(29.13±5.11)scores,lower than that of patients without self-care ability(33.99±4.32)scores,the differences were statistically significant(P<0.05).The APGAR score for advanced patients with chest obstruction was(6.32±1.44)scores,the BIPQ score was(62.75±6.37)scores,the DASS score was(51.08±4.19)scores,and the FPP score was(2.63±0.74)scores,PCST was negatively correlated with APGAR(r=-0.759,P<0.001),positively correlated with BIPQ(r=0.722,P<0.001),DASS(r=0.741,P<0.001),and FPP(r=0.751,P<0.001).Multiple linear regression analysis showed that age,self-care ability,APGAR,BIPQ,DASS,and FPP were all influencing factors of the degree of hospice care needs in advanced patients with chest obstruction(P<0.05).Importance matrix analysis indicated that the scores of APGAR,BIPQ,DASS,and FPP were all in the priority improvement area;age and self-care ability were in the secondary improvement area.Conclusion The overall demand for hospice care among advanced patients with chest obstruction is relatively high,which is influenced by multiple factors such as age,self-care ability,frailty,family functioning,psychological state,and disease perception.Clinically,guided by the importance matrix,priority should be given to improving family support and psychosocial services.
