Effect of Constraint-Induced Movement Therapy Combined Task-Oriented Training on Upper Limb Function in Children with Spastic Hemiplegic Cerebral Palsy

Abstract:Objective To explore the effectiveness of constraint-induced movement therapy(CIMT)combined task-oriented training in improving upper limb function in children with spastic hemiplegic cerebral palsy.Methods Sixty-four children with hemiplegia who met the inclusion criteria were enrolled and randomly assigned to the control group and the experimental group using the random number table method,with 32 cases in each group.The mean age was(50.34±9.18)months and(52.79±8.91)months,respectively.Assessment was performed again after 3 months of intervention.In the control group,2 cases were lost to follow-up and 1 case did not complete the course;in the experimental group,1 case was lost to follow-up and 2 cases did not complete the course.Finally,29 cases in each group were included in the study.The control group received conventional rehabilitation training,while the experimental group additionally received CIMT combined task-oriented training.The Fugl-Meyer assessment(FMA),Peabody developmental motor scales-fine motor(PDMS-FM)and functional independence measure for children(Wee-FIM)were used to evaluate the motor function,coordination function and functional independence of the affected upper limb before and after treatment.Results Before treatment,there were no significant differences in FMA,PDMS-FM and Wee-FIM scores between the two groups(P>0.05).After treatment,the FMA score[(28.28±2.37)points],PDMS-FM score[(104.55±6.83)points]and Wee-FIM score[(54.91±2.62)points]of the control group were higher than those before treatment[(25.79±4.44),(97.07±6.54)and(36.91±2.62)points],and the differences were statistically significant(P<0.05).The FMA,PDMS-FM and Wee-FIM scores of the experimental group[(30.21±5.36),(120.03±6.99)and(65.58±2.59)points]were higher than those before treatment[(24.00±4.74),(96.31±7.07)and(36.59±2.59)points]and those of the control group after treatment,and the differences were statistically significant(P<0.05).Conclusion CIMT combined task-oriented training can promote the recovery of upper limb motor function in children with spastic hemiplegia,thereby improving their ability to perform activities of daily living.
