Abstract:Objective To investigate the intraoperative,in-hospital postoperative outcomes, and mid-to-long-term results of using radial artery grafts in coronary artery bypass grafting for patients with multi-vessel coronary artery disease and diabetes mellitus. Methods A total of 489 patients who underwent isolated coronary artery bypass grafting using radial artery grafts at Anyang District Hospital of Puyang City from January 2018 to January 2025 were enrolled. They were divided into two groups based on diabetes status:the observation group(with diabetes mellitus,n=202)and the control group(without diabetes mellitus,n=287).Baseline characteristics, intraoperative and in-hospital postoperative outcomes,30-day follow-up results, and graft patency were compared between the two groups. Results Comparison of baseline data showed that the observation group had a higher incidence of myocardial infarction than the control group(x2=4.378,P=0.036),while no statistically significant differences were observed in other baseline characteristics.Regarding intraoperative and in-hospital outcomes, the observation group exhibited a higher proportion of new-onset atrial fibrillation(x2=7.861,P=0.005).No significant differences were found in operative time or blood product utilization rates(P>0.05).At the 30-day follow-up,the incidence of cerebrovascular events was higher in the observation group(x2=4.425,P=0.035).No statistically significant differences were observed in major adverse cardiac and cerebrovascular events or all-cause mortality(P>0.05).Patency rates of the left internal thoracic artery and radial artery grafts showed no significant intergroup differences(x2=0.272,P=0.602;x2=0.378,P=0.539),while saphenous vein graft patency was significantly higher in the observation group(x2=5.070,P=0.024).Conclusion For patients with multi-vessel coronary artery disease and diabetes mellitus, the rational use of radial artery grafts in coronary artery bypass grafting can still achieve favorable clinical outcomes and mid-to-long-term prognosis.
