Relationship Between High-Density Lesions and TEG Parameters on CT Plain Scan of the Head After Intravascular Recanalization in Acute Ischemic Stroke and Bleeding Transformation and Prognosis

Abstract:Objective To explore the relationship between high-density lesions on CT plain scan and thromboelastography(TEG)parameter characteristics,as well as bleeding transformation and prognosis after intravascular recanalization in acute ischemic stroke.Methods A retrospective analysis was conducted on the clinical data of 318 patients with acute ischemic stroke treated in Anyang Third People's Hospital from January 2020 to January 2023.All patients were given intravenous thrombolytic therapy.The clinical data,CT and TEG parameters of patients with and without hemorrhagic transformation,good prognosis and poor prognosis were analyzed.Results The NIHSS score of patients with hemorrhagic transformation at admission was higher than that of patients without hemorrhagic transformation(P<0.05).The proportion of patients with frosted-like high-density shadows and diffuse high-density shadows in patients with hemorrhagic transformation was higher than that in patients without hemorrhagic transformation(P<0.05).The clotting reaction time(R)and blood cell agglutination clot formation time(K)values of patients with hemorrhagic transformation were higher than those in patients without hemorrhagic transformation(P<0.05).The blood cell agglutination clot formation rate(Angle),maximum clot intensity(MA),and clotting index(CI)values of patients with hemorrhagic transformation lower than those in patients without hemorrhagic transformation(P<0.05).Logistic regression results showed that frosted-like high-density shadows,diffuse high-density shadows,and R value were the influencing factors of hemorrhagic transformation in patients(P<0.05).The age and NIHSS score at admission of patients with poor prognosis were higher than those of patients with good prognosis(P<0.05).The proportion of patients with diffuse high-density shadows in patients with poor prognosis was higher than that in patients with good prognosis(P<0.05).The R and K values of patients with poor prognosis were higher than those in patients with good prognosis(P<0.05).Logistic regression analysis showed that age and NIHSS score at admission were the influencing factors of prognosis in patients(P<0.05).Conclusion The high-density CT plain scan and TEG parameters of the head after intravascular recanalization are influencing factors for hemorrhagic transformation in patients with acute ischemic stroke,but they are not significantly related to the prognosis of patients,and are worthy of further research.
