Clinical Characteristics of Septicemia in Premature Infants Caused by Different Gram Positive Coccus

Abstract:Objective To investigate the clinical characteristics and risk factors of sepsis in preterm infants caused by different Gram positive(G+)coccus.Methods Preterm infants diagnosed with G+coccal septicemia in the neonatal intensive care unit(NICU)of the Third Affiliated Hospital of Zhengzhou University from January 2019 to December 2020 were retrospectively divided into coagulase negative Staphylococcus(CoNS)group and other G+coccus group according to bacterial species.The high risk factors of the mothers during pregnancy and the clinical characteristics of the children were analyzed respectively,and the high risk factors and characteristics of different G+coccal septicemia in preterm infants were discussed through comparative analysis.Results Forty-four preterm infants with G+coccal septicemia were included in the study,including 23 cases in CoNS group and 21 cases in other G+coccus group,accounting for 52.27%,47.73%of preterm septicemia patients during the study period.The comparison of G+coccal septicemia between the two groups showed that the time of central vein insertion,mechanical ventilation time,temperature instability,feeding intolerance,and the time of first blood culture positive detection were statistically significant(P<0.05).Binary logistic regression analysis:birth weight,parenteral nutrition,invasive mechanical ventilation,premature rupture of fetal membrane≥18 h were independent risk factors for G+coccal septicemia(P<0.05).Birth weight 1 000-<1 500 g,central vein catheterization,invasive mechanical ventilation,nasal feeding,and Ⅲ degree amniotic fluid contamination were independent risk factors for CoNS septicemia(P<0.05).Conclusion Birth weight,invasive mechanical ventilation,central vein catheterization and nasal feeding are independent risk factors for neonatal CoNS septicemia.With the improvement of NICU treatment technology,the clinical characteristics of different G+coccus are different,which should be paid great attention to.
