Clinical Retrospective Analysis of 150 Hospitalized Patients with Bullous Pemphigoid

Abstract:Objective To retrospectively analyze the clinical data of patients with bullous pemphigoid(BP),and to study the correlation between relevant laboratory indicators and BP severity.Methods The blood index of 150 patients with BP were collected and compared with those of 100 healthy controls.BP patients were divided into three groups:mild,moderate and severe,comparison the differences among the groups were performed.Spearman correlation analysis was used to assess the correlations between anti-BP180 antibody levels and other laboratory indicators.Receiver-operating characteristic(ROC)curve analysis was used to compare the diagnostic efficacy of neutrophil to lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR)and single parameter neutrophil(NEU),lymphocyte(LYM),platelet(PLT)for BP and calculate the cut-off value,sensitivity and specificity.Results Compared with the control group,the levels of white blood cell(WBC),eosinophil(EOS),NEU,NLR,PLR,D-dimer in BP patients were increased,while the levels of LYM,serum albumin were decreased(P<0.05).The comparison among subgroups of BP patients showed that NLR,the anti-BP180 antibody,and D-dimer were severe group>moderate group>mild group,and serum albumin level was severe group<moderate group<mild group(P<0.05).The EOS level in severe group was higher than that in moderate and mild groups(P<0.05).PLR,immunoglobulin E(IgE)in mild group were lower than those in moderate and severe groups(P<0.05).Spearman correlation analysis showed that anti-BP230 level,NLR,PLR,PLT,EOS,IgE,D-dimer and anti-BP180 antibody levels were positively correlated,while albumin and anti-BP180 antibody levels were negatively correlated(P<0.05).ROC curve showed that the area under the curve was NLR(0.943)>NEU(0.919)>LYM(0.798),PLR(0.811)>LYM(0.798)>PLT(0.567).The optimal cut-off value of NLR was 2.55,the sensitivity of 90.7%,the specificity of 90.0%.The optimal cut-off value of PLR was 168.01,with a sensitivity of 69.3%and a specificity of 84.0%.Conclusion The levels of NLR,anti-BP180 antibody,D-dimer and serum albumin were related to the severity of BP,PLR,EOS and IgE are related to the severity of BP to a certain extent.The diagnostic efficiency of NLR and PLR for BP is better than that of the corresponding single parameter.
