Effect of VRD Regimen Combined with Autologous Hematopoietic Stem Cell Transplantation on Newly Diagnosed Multiple Myeloma and Prognostic Factors

Abstract:Objective To investigate the effect of bortezomib,lenalidomide,dexamethasone(VRD)regimen combined with autologous hematopoietic stem cell transplantation(ASCT)in the treatment of newly diagnosed multiple myeloma(MM)and analyze the prognostic factors.Methods A retrospective analysis was conducted on the clinical data of 108 newly diagnosed MM patients admitted to Nanyang Central Hospital from January 2017 to September 2022.The clinical efficacy of ASCT before and after transplantation were calculated.According to the disease status before and after transplantation,patients were divided into complete remission(CR)/very good partial remission(VGPR)group and partial remission(PR)group,and the relationship between the efficacy and prognosis before and after ASCT transplantation was analyzed.Clinical data between the death group and the survival group were compared,and Cox multivariate regression was use to analysis to identify risk factors affecting the prognosis of MM patients.Results All patients successfully reconstructed their hematopoietic function after transplantation,and there were no transplant related deaths.The proportion of CR and VGPR after transplantation was higher than before transplantation(P<0.05).The survival rates of the CR/VGPR group before and after transplantation were higher than those of the PR group(x2=9.393,8.369,P=0.002,0.004).The age and risk stratification of the death group were classified as high-risk proportion,diagnosis to transplantation time,pre transplant clinical efficacy as PR proportion,post transplant clinical efficacy as PR proportion,and early recurrence proportion were higher than those of the survival group(P<0.05).Cox regression analysis showed that age(OR=1.063,95%CI:1.016-1.113),risk stratification(OR=3.913,95%CI:1.837-8.337),and early recurrence(OR=3.656,95%CI:1.693-7.895)were related factors affecting the prognosis of newly diagnosed MM patients(P<0.05).Conclusion The combination of VRD regimen and ASCT in the treatment of newly diagnosed MM patients can improve clinical efficacy.The clinical efficacy of CR and VGPR before and after transplantation can improve patient's survival rate.Age,risk stratification,and early recurrence are related factors that affect the prognosis of newly diagnosed MM patients.
