Comparison of the Effects of Afentanil and Remifentanil in Anesthesia for Thyroid Surgery

Abstract:Objective To compare the anesthetic effects of afentanil and remifentanil in patients of thyroid surgery.Methods The study subjects included 92 patients of elective thyroid surgery in the Second Affiliated Hospital of Zhengzhou University from March 2022 to December 2023,and were randomly divided into afentanil group and remifentanil group,with 46 patients in each group.Afentanil was used in both induction and maintenance of anesthesia in afentanil group,and remifentanil was used in remifentanil group.The intraoperative hemodynamic indexes,postoperative visual analogue scale(VAS)scores,anesthesia recovery indexes and perioperative adverse reactions were compared between the two groups.Results The mean arterial pressure(MAP)and heart rate(HR)immediately after endotracheal intubation(T,)in both groups were significantly higher than those before anesthesia induction(T0),and the MAP and HR were gradually decreased at the time point of 30 min after operation(T2)-the end of operation(T3),the MAP and HR in afentanil group at the time point of T,-T3 were significantly lower than those in remifentanil group(P<0.05).There was no statistically significant interaction effect between the two groups of MAP and HR(P>0.05).The VAS scores in afentanil group at the time points of 30 min,1 h and 2 h after extubation were lower than those in remifentanil group(P<0.05).There was no statistically significant interaction effect between the two groups of VAS scores(P>0.05).The respiratory recovery time,consciousness recovery time and extubation time in afentanil group were significantly longer than those in remifentanil group(P<0.05).The incidences of perioperative respiratory depression and hyperalgesia in afentanil group were significantly lower than those in remifentanil group(P<0.05).Conclusion The application of afentanil in anesthesia induction and maintenance of thyroid surgery patients has more advantages in stabilizing hemodynamics,alleviating postoperative pain and reducing the risk of adverse reactions,while remifentanil can make patients wake up faster after surgery.
