Abstract:Objective To analyze the changes of miR-122 levels in patients with chronic liver failure(CLF)complicated with bacterial infection and its prognosticative value. Methods A total of 106 CLF patients with bacterial infection admitted to Henan Provincial People's Hospital from June 2021 to June 2023 were selected as the infection group, and 120 patients with simple CLF admitted during the same period were selected as the CLF group. In the infected group, patients were divided into a survival group(61 cases)and a death group(45 cases)based on their survival results within 30 days.Serum testing was conducted to quantify the expression levels of miR-122 in both groups of patients. To reveal the factors that may affect the clinical prognosis of CLF patients with bacterial infections, univariate and multivariate logistic regression analyses were conducted.Using receiver operating characteristic(ROC)curve analysis, the predictive value of serum miR-122 in the clinical prognosis of CLF patients with bacterial infections was evaluated. Results The relative expression level of miR-122 in the serum of patients in the infection group was higher than that in the CLF group(P<0.05).ROC curve was used to analyze the predictive value of miR-122 for CLF combined with bacterial infection, with an area under the curve of 0.955.In patients with CLF combined with bacterial infection, the relative expression level of miR-122 in the serum of the deceased group was higher than that of the surviving group(P<0.05).The results of univariate and multivariate analyses indicated that model for end-stage liver disease(MELD)score, alanine aminotransferase(ALT),aspartate aminotransferase(AST),and miR-122 were all associated with the prognosis of CLF patients with bacterial infections(P<0.05),and were independent risk factors.ROC curve analysis was used to evaluate the prognostic value of serum miR-122 in this patient population, and the area under the curve was found to be 0.772.Conclusion Serum miR-122 expression is abnormally elevated in patients with chronic liver failure complicated by bacterial infection, and the degree of miR-122 elevation is closely related to poor prognosis of patients. It is a potential biological indicator for predicting clinical prognosis of patients.
