Effects of Lidocaine Combined with Dexmedetomidine on Stress Response and Recovery Period of General Anesthesia in Neurosurgical Patients

Abstract:Objective To investigate the effects of lidocaine combined with dexmedetomidine on stress response and recovery period of general anesthesia in neurosurgical patients.Methods The study subjects included 90 neurosurgical patients of elective surgery in the Second Affiliated Hospital of Zhengzhou University from January 2023 to November 2024,and were randomly divided into dexmedetomidine group(group D)and lidocaine combined with dexmedetomidine group(group LD),with 45 cases in each group.Group D was given dexmedetomidine 0.5 μg·kg-1 by slow pump for 15 min before anesthesia induction,and then pumped continuously at 0.4 μg·kg-1·h-1 until the end of surgery.Group LD was given lidocaine 1.5 mg·kg-1 by single injection and dexmedetomidine 0.5 μg·kg-1 by slow pump for 15 min before anesthesia induction,and then continuously pumped at 1.5 mg·kg-1·h-1 and 0.4 μg·kg-1·h-1 until the end of surgery,respectively.The mean arterial pressure(MAP)and heart rate(HR)were recorded at time points of 10 min of entry and immediately after extubation.The concentrations of epinephrine and norepinephrine were detected at the time points of pre-operative and post-operative 24 h.Spontaneous eye opening time,extubation time,orientation recovery time and the number of cases of choking coughing and restlessness during the recovery period were recorded.Results Compare with 10 min of entry,MAP and HR in both groups were higher at time point of immediately after extubation,but the group LD was lower(P<0.05).At post-operative 24 h,serum epinephrine and norepinephrine were increased in both groups,but the indexes in LD group was lower than D group(P<0.05).The spontaneous eye opening time,extubation time,orientation recovery time,extubation time and orientation recovery time of group LD were shorter than those of group D.The incidence of choking cough during the recovery period in the group LD was lower than that in the group D(P<0.05).Conclusion Lidocaine combined with dexmedetomidine has more advantages in maintaining hemodynamic stability and inhibiting stress response in the anesthetic application of neurosurgical patients,which can significantly shorten the postoperative recovery time and reduce the risk of adverse reactions during the recovery period.
