Effect of the Change of Triglyceride/High-Density Lipoprotein Cholesterol Ratio on Macrosomia in Pregnant Women with Diabetes During the Third Trimester of Pregnancy

Abstract:Objective To observe the change of the ratio of triglyceride(TG)/high-density lipoprotein cholesterol(HDL-C)in the third trimester of pregnancy in pregnant women with gestational diabetes mellitus(GDM),and to analyze its effect on macrosomia.Methods This study was a prospective study.GDM pregnant women who were examined in Henan General Hospital from April 2021 to April 2022 were continuously collected as the research objects.All pregnant women received standardized treatment.According to the occurrence of macrosomia after delivery,they were divided into macrosomia group and non macrosomia group.The changes of TG/HDL-C ratio at the time of enrollment,32 weeks of pregnancy,34 weeks of pregnancy,and 36 weeks of pregnancy were compared between the two groups,as well as other general data at the time of enrollment.Logistic regression was used to analyze the influence of TG,HDL-C and TG/HDL-C ratio on the occurrence of macrosomia in late pregnancy of GDM pregnant women,and the predictive value of TG,HDL-C and TG/HDL-C ratio on the occurrence of macrosomia in late pregnancy of GDM pregnant women was analyzed by receiver operating characteristic curve.Results Totally 220 pregnant women with GDM were included,17 of which were excluded during follow-up.Finally,203 completed the study,including 21 cases of macrosomia,the incidence of macrosomia was 10.34%.The fasting plasma glucose level of the macrosomia group was higher than that of the non macrosomia group,with a statistical significant difference(P<0.05).The levels of TG,TG/HDL-C at 32 weeks,34 weeks and 36 weeks of gestation showed an upward trend and the level of HDL-C showed a downward trend when the two groups of pregnant women were enrolled in the group,but the changes in the non macrosomia group were smaller,with a statistical significant difference(P<0.05).The levels of TG and TG/HDL-C in the macrosomia group were higher than those in the non macrosomia group at different time points,and the levels of HDL-C were lower than those in the macrosomia group,with a statistical significant difference(P<0.05).Logistic regression analysis showed that TG and TG/HDL-C ratio were the risk factors of macrosomia in GDM pregnant women(OR>1,P<0.05).HDL-C was a protective factor for macrosomia in GDM pregnant women(OR<1,P<0.05).ROC curve showed that the AUC predicted by TG and HDL-C in late pregnancy of GDM pregnant women was 0.664 and 0.698 respectively,and the AUC predicted by TG/HDL-C ratio was 0.713.Conclusion The ratio of TG/HDL-C in the third trimester of GDM increases with gestational age,and the increase of the ratio of TG/HDL-C will increase the risk of macrosomia.
